Docetaxel.

There's evidence that's where you should have started. Of course that evidence was not available when you did start. Jevtana is also an option and I think Nick would tell you to go that way. So would Maha, Ian, and many more involved with the Taxan trials. The reason? Because you need your fingers for your guitar work. JEVTANA does not have the hair loss and neuropathy issues that docetaxel has...at least not to the same degree. That stated, I would still say docetaxel. Taking precautions like "ice gloves" and such are known to be effective.

You could also go on one of the PD-1/PD-L1 trials. however some of those are restricted because you under went a previous immunotherapy.

There's a few other trial options out there too.

You know I love ya, brother! PD-1, PD-L1 trials. But if it were me I'd look closer at the taxans. Just saying. You have already been on "chemo" drugs. Pill form. With cortical steroids. And your response to therapies across the board has been fantastic. I don't think it will be any different with docetaxel. And we have seen cases that 2nd line hormonal AR drugs start working again after docetaxel.

Todd, have you already had a run with Xtandi? Sorry, I can't remember and dont see it in your signature. I was on that for a while...
And you can still play guitar during and after chemo. I did and I do. Still have neuropathy in hands but can move them along the strings just fine.
