Prep H is useless. Get the suppositories the doc can superscribe (hydrocortisone). Sitz baths, and medicated wipes can also help. Also is he constipated? That will make things worse so a stool softener might be in order. Lots of fiber and no straining during a bowel movement make a difference.
That's about all you can do for home treatments. If they're really bad, they can do some laser and other treatments.

I've had 'em bad off and on my whole adult life. It wasn't until the doc prescribed hydrocortisone that I found any real relief.
It comes in a creme as well (which is what I use). Should provide some relief. (It always has mine).
