Yes, it happens...
It was me who peed through a strainer to figure out what those dark specks were. They were bits of suture material.
When the surgeon sutures the ends of the urethra together (s)he uses absorbable sutures that will be attacked by the body and absorbed in six weeks or so. The stitches go all the way through the wall of the urethra and part of one side of the stitch is exposed to the flow of urine. When your body has finally attacked the stitch sufficiently for it to pop open (well after the joined tissues have healed together) then you can pass some very small clots and bits of suture material. The loose ends of the stitch can be a bit irritating and sometimes you may see a drop or two of blood.
