I assume you have a "cancer" policy as opposed to your regular health insurance. These usually do not provide coverage for early stage low risk cancers. In situ, basically means "in its place", denoting no spread. Generally this is a very early, non aggressive cancer that has just started developing and has not presented any symptoms.
Many on the forum have received payments for PCa with cancer policies. I would think a G6 with the PSA progression he experienced would be covered, but it will depend on the contract language and the actual dx.
In your situation, I agree that LDR/HDR brachytherapy may be appropriate. I can vouch for HDR and would be happy to provide you with information from my experience. Are you considering Active Surveillance? It appears to be relatively low risk and you should have many options and no reason to rush into a treatment plan.
