Good day

Thank you once again to all the insight, guidance and information given on this forum. Especially all the guidance at the beginning.

I did not know a lot about prostate cancer before the diagnoses, but since then I have read 3 books busy with another at present and it feels I know even less since there is so may opinions. I have filtered all the information and tried to look at the biggr picture.

I have taken all the information into consideration including my personal situation and a few other factors and have decided to continue with the radical prostactimy.

Robotic booked for 3 July 2016

I have never been good with regrets , regret is almost like cancer, maybe sometimes even worse. There is nothing you can do to the situation that cause you to regret someting, since that is looking back. You need to work with the present and look at the future.

So it appears my best option is the operation and I will deal with what is laying ahead.

below my histolgie report

diagnosed - age 43
april 2016

MACTOSCOPYy
(1) left: Multiple needle core biopsies of <1 to 20 mm in length.
(2) Right: Multiple needle core biopeies of <1 to 18 m in 1ength.

Microscopy:
(1,2) Microscopic examination of three of the biopsies from the left lobe and
one of the biopsies from the right lobe and with involvement of respectiwely 40
and 80% of the surface area of the involved biopsies, shows the presence of an
infiltrating adenocarcinoma, cleason score 7 (3+4). There are focal areas of
high grade prostatic intraepithelial neoplasia. There are no signs of
perineural or vascular involvement. The background appearance is that of
limited benign nodular hyperplasia. inflammatory features appear unremarkable.

coment

(1.2) the appearance is that of an infiltrating adenocarcinoma, cleason score 7
(3+4 ) .

Deon124, Your decision to have RRP seems like the best choice for you. Delaying treatment or trying focal treatments would not be recommended. Good luck and best wishes for your recovery and being cured.

You aren't alone in feeling that way. It seems that the only thing that is true all the time about prostate cancer is that you really don't want it.

I wish you well with the surgery and recovery and all your future PSA tests indicate that your prostate cancer was stopped in its tracks.

Good luck Deon, we're all behind you. And will be waiting for you when you get out of surgery. It sounds like you've done a lot of self education, and no doubt you've made the right decision for yourself

Deon, I want to wish you the best on your upcoming surgery and give you my outcome after RALP on 3/21/16.
Was home the next day after surgery and no pain what so ever. Only soreness. Cath was no problem and was out after 7 days. Incontinence as expected, but now 3 months later only drips during daytime, dry at night. You will need a lax and stool softner to get and stay regular. ED recovery is on track, not like it once was, but enjoyable for both, with high hopes for the future. Do your kegels and walk. Bottom line, my surgery could not have went any better and my recovery has been great so far. I wish the same for you, and better.

1 days after surgery. All went reletively well. Will have to wait for the final report.
LEFT side 100% nerve spared, right side 40% .
Apparently very little bleeding.

numbness in right leg and pain in lower back when i lay down but nothing when I am upright. After the op I had a lot of pain, but it subside today with flare ups every now and again
Taking short walks but its difficult.

Deon, welcome to the other side. I'd encourage you to drink lots of water to keep things flushed out, and get up and walk at least a few steps every hour. Make sure you take your stool softeners and stick with the high fiber diet for a bit. Your gut will wake up in the next day or two, and the important thing is to not let yourself strain when you try to go.

In a week or so, the catheter and sutures will come out, and then recovery really begins.

Keep up the walking, even if it's just laps around your chair every few hours. It really helps.

Dang, Andrew beat me to it, I was going to day it gets better each day also.

Keep walking a some every day and drink a lot of water as the others have said.

The next milestone is your first BM. The weird things we celebrate while dealing with PCa. LOL

Good luck with your pathology report. If you want, you can ask for your surgical report also either now or later on. It tells you what they did when you were in LA LA Land.

Thankyou . I will make sure I ask both reports . So far its up and down. One moment I feel good and the next I am tired and sore. I know it is early days and 6 weeks to go. Looking fwd to Tuesday for the removal of the catheter, and the details of the report.

40% nerves spared on right and 100%left. What does this mean for errections down the line? any hope or should I consider moving to Tibet and become a monk.

NO!
don't move to Tibet. You're chances of recovery are good. Even those with poor chances never know. DH had non nerve sparing.
And a year and a half later he is getting erections with a pill. A couple other guys on HW with non nerve sparing have also been so lucky.

Deon, a common post-surgical protocol is to start on Viagra or Cialis, daily, after catheter removal. This helps get the blood flowing in the area and helps the healing process. There is also evidence that Cialis in particular can help continence as well. Ask at the catheter removal visit. If you didn't have any issues pre-surgery, you've got a reasonable chance of recovering that functionality.

Any recomendations on specific type of scans to do looking at below results? I have to make the decition on my own so will not held anybody liable for suggestions.

- tumure involves 40% of the prostate gland
-extensive perinuural invasion present
-tumar present at the apax,left lateral and posterior margins
-extraprostatic extension present
-high grade pin present
-mild chronic prostatitus present

# left and right seminal vesicles
-within normal limits
-no invasive malignancy present

primary tumar pt3a: extraprostatic extension

margins involved by invasive carcinoma : multifocal, apical, left lateral and posterior

Hi Deon. Welcome to the other side, and sorry for the less-than-spectacular post op pathology.

I have seen where similar situations have typically been ADT + SRT. Before all that, you might want to have some relevant facts available, ie: current PSA and see how much it changes over a time period.

Surely TA and some others with a better experience base will also give some info shortrly.

the doctor suggedted I must only go for a PSA test 6 weeks after the op?

My surgery was 2 days after yours - I'm 48
Recovery sounds similar. I got out for a 1 mile walk yesterday, and will go again today. Like you, waiting to get the catheter out!
