My friend's rising psa

Just wanted any thoughts on this----
My friend had a divinci RP from a very experienced surgeon at City of Hope. The pathology was good, clean margins, capsule contained etc--this was about 4 years ago? Then after his psa was .06- 3 moths later .08- 3 months later .16- 3 months later .22 . so.... a year after rp they irradiated the prostate bed....all scans showed nothing moving to other parts of the body.....his psa then got down to .1 3 months after rad and then down as low as .02, then bounced around something like .03 to .04 for a few years--- It stayed there til December last when he got the worst UTI he ever had and a hernia operation---now, 4.5 months later his psa is .15 I mention this because I am hoping that this may have spiked his psa and not ca. The doc has ordered a new body scan and hen a new blood test in a month or so...

Does anyone have any thoughts on this case and also about the idea that the rise could be an anomaly???

So, he'll probably need to get used to the wait-and-see approach to managing his disease. While the upcoming tests will tell him more, it is starting to sound like his treatments failed to cure his disease and he will need to manage it going forward. That's a disappointment for him -- kinda sucks. That's the bad news. The good news is that with the relatively slow advance of his PSA, years after his treatment was finished, his disease sounds pretty manageable and his prognosis is probably pretty good.

Do you know the Gleason score reported for his post-op pathology report? That might give him some idea how fast he needs to wait.
