First congratulations on making it to 87!! And you are also very active, kudos to you!!

If you are enjoying life which is what it sounds like you are doing, I'd keep monitoring your PSA and enjoy life.

Sorry about your diagnosis, but great to hear how active you are! I think you'd be totally on base if you were to do nothing.

When you say you didn't want shot therapy, do you mean you don't want to do hormone therapy? That would be the easiest thing to do if you chose to treat it. Or, just wait and see what happens and if the PC becomes an issue, go on hormone therapy then.

I'm 65, and my immediate reaction was, he'll, just forget it and enjoy each day as best you can. But then I thought to myself, "I'm guessing, when I'm 87 (if I get there) I'll be hoping to live another 10-15 years with good QOL.

So I think Bololidat is correct.you (only you) know the answer.
Of course that nomogram that Allen posted could be helpful.

Plugging your numbers into the normogram can be quite an eye opener. Assuming total health which you appear to have, your chance of dying of PC in the net 10 years is probably between 4% and something well less than 10%. Your odds of having died of something else are vastly higher, and that is assuming otherwise perfect health on your part. If you have even 1 health issue, that just increases your odds of something else getting you first. That actually is the same for most men with PCa, though the numbers will not be near as lopsided for a man in his 60s or 70s.

And in your case, exercise has frequently been shown to be one of the top factors that lowers your chance of dying of PC by 25-65%(all depends on which study of course), and you are already all over that part.

Of course, I suppose just not dying of PC is not the same as not suffering from PC. Like my URO told me, living with advancing PC is no picnic. OTOH, the treatments for PC are not uncommonly no picnic either. My 80 something(84?) neighbor across the street, whose wife died of Alzheimer's a few years ago, continues to ignore his PCa and his rising PSA of about 20. Always working in his yard, traveling, going to see his lady friends, to church, walking the 2 mile neighborhood course. No symptoms so far, just the diagnosis. He has opted to let nature take it's course, so far at least. One thing to his advantage these last several years is that he has had no onerous side effects from treatment for PCa.

Of course surgery is out of the question. Age 70 is the cut off age that most all responsible surgeons follow for RP.

Hormone therapy comes in many varieties. You could use Casodex and Avodart and see what effect it has on PSA. These have minor side effects for most men, nothing like the ADT drugs (Lupron, Eligard, Firmagon, etc.).

Both SBRT and HDR brachytherapy can be used with minimal side effects. You present a higher risk than is desired for HDR mono-therapy, but it could be considered if you are looking to slow the disease down and it hasn't already spread. The scans may help your decision. I wouldn't rule out treatment due to age and would definitely consider it based on the G8. If it gets nasty the PCa side effects will be worse then the ADT side effects used to slow it down.

Im not even trusting the gleason score...seems suspiciously convenient...as to invite treatment from medico's...if...and I don't suggest this...if you were to go for tx...get the gl score slides re read by bostwick or epstein...before any else is done. No pni tells me what ever it is...has not been there long...my sense is you have indolent pca and got it late in life...I don't care if you live to a 100...Pca isn't gonna kill ya...at your age...treatment may not kill ya but might make you wish it would...At the least tx will kill,arguably, your spirit and make you old ahead of your time... rejoice in your life and as you say... let nature take its course...nature has been pretty faithful up to now... The fact that this is even intruding on a very full and happy life...due to to this diagnosis...ticks me off... Im 73...fit to a fault... and plan on living large and long...and if pca gets me in the end...I'll give the devil its pound of flesh...my 28grams of spirit will remain mine...till the end of time...ps have your medico read this post and watch him as he reads... naw...maybe not...but you get the point...

Your cancer is aggressive, I think you should not ignore it. You can try antiandrogens (Casodex tablet) with minimal side effects
