One of the great benefits of living in Australia is that the custom of tipping is virtually unknown. Although it may occasionally be offered (for particularly outstanding service) it is never expected.
The closest we come is waving aside any change that may be due.
The thought of tipping anyone at a health facility, particularly one specialising in cancer patients, seems a bit much to me.
Of course the tipping issue does cause Aussies great confusion (and sometimes embarrassment) when we visit the US. I know I forgot to leave a tip in a restaurant in LA once and felt bad when I saw my dinner companions surreptitiously leaving extra cash on the plate. I was appropriately apologetic.
