Hello, I am 46 years old and have had a scare about my PSA. I have it checked every six months because I have history of prostate cancer on both sides of the family. My doctor contacted me last week and told me my PSA had jumped from 1.5 to 6.4, so we did another test yesterday and it is down to a 3. I meet with him on Tuesday. What is your advice moving forward. He is my DO and I have not seen a urologist. Thanks.

Hi Casey. The fact that it came way back down that far is probably a good sign. 3 would probably still be slightly high for your age. But hopefully it's still on it's way down from whatever caused it to spike. I would think another psa test soon would be good before I jumped to any conclusions. What does doc say?

I meet with him at 815 on Tuesday and he is my regular DO. Thanks for your input.

Welcome to HW.

Sounds more like prostatitis. PSA in PC doesn't drop like that. PSA with wild swings is more likely due to prostatitis. A retest in a few months would be in order.

If you have concerns, you might get a referral to a URO.

It's a scary swing, and should be monitored, but not likely to be caused by PC.

As the others have noted, a psa spike and lowering is far more indicative of prostatitis than prostate cancer.

Certainly, you do not have cause yet for a biopsy, but some more comprehensive and accurate blood tests like PHI or 4K Score would provide you and your doctor with better data than psa alone.

Given your family history and your PSA, it might be a good idea to consult a urologist.

Maybe you already know this, but it wont hurt to mention. Blood test before DRE and no ejaculation 2 to 3 days before blood test for PSA. Its reported that both will raise PSA on tests. Just a thought. Best of luck
