OK so now I am seriously leaning towards RT of some form.
Due to my location and other general life situations a 8-9 week course of treatment is all but out.
I am looking at HDR monotherapy or SBRT. I have pretty much done the HDR reasearch and feedback from you guys. Now I would like to do the SBRT.
Monday the 21st I travel to Seattle to meet with Dr. Jay Lilao RO at UWMC and I would like to be fully prepared on these two treatments. Early this week I met with Dr. Maestas RO in Spokane and discussed IMRT with her. That is the only style she does. Again the travel and/or temporary living near the treatment site is almost out of the question particularly if another treatment with similar outcome percentages is available.

So please SBRT stories and info please.

If I had been aware of SBRT at the time of my DX, I would have pursued it and used it. At the time, my local ROs only discussed LDR brachy and IMRT.

Okay, not exactly experience, but just a vote for seriously considering it.

In Seattle, the SBRT expert is Robert Meier at Swedish Hospital. Below is an article about a very impressive clinical trial he recently presented. 5-year cancer control was 97% for intermediate risk patients.

As for my personal experiences, I was impressed with the combination of effectiveness and low SE for SBRT as a mono therapy, especially for my situation (G6, normal size, shape, MRI showed no lesions, etc) as there was a good chance the cancer had not escaped the capsule. But I did not want to pursue AS or wait too long due in making my Tx decision to my family Hx and because two of the cores were 80% affected.

If you are looking for info on personal experiences on the effectiveness of SBRT, I would defer to other members as I just completed my Tx recently and don't have any experiences in that area to share yet. Tall Allen helped me a lot in making my decision and would defer to him on the research done on effectiveness of SBRT.

I chose SBRT at Cleveland Clinic since the data I looked at suggested it had the least side effects with equal to or better cancer control than all the other options for my Dx. Two months post treatment and no side effects except a smaller amount of ejaculate.

5's good afternoon, Like you I was a 4+3 and chose SBRT. I was part of a clinical trial at UAB in Birmingham Al. I finished in Nov of 2016 5 treatments with a intergrated boost to the actual tumor. My RO told me that standard of care for unfavorable intermediate risk included HT but he but he left it up to me. I chose to go with the standard of care. My treatments were about 15-20 min. long each from the time I got on the table until I got off. The equipment was a Varian Edge. The first 2 treatments were no problem but after the 3rd I experienced 24hrs of pure hell. I had to go every 20min and it was like trying to pee a teaspoon full of volcanic lava. That went away after about 24 hrs. and I have not had any other adverse SE's since. I took 2 3mo shots of Lupron the last on Jan 19 2017. My PSA last month was 0.05 and my T level was back up to 154. At this time I can't help but believe I made the right choice but as we all know only time will tell. I was also on Flomax up until about a month ago and I said enough is enough and quit I believe the Lupron and Flomax caused more bad SE's than the SBRT. I am glad to say that all parts work and work well and the desire for it to work is coming back also. I wish you good luck in your journey.

Count me as a satisfied SBRT patient. The five treatments were fast, simple and nearly doze-off-inducing. No side effects for six months, and then some increased urinary difficulties call a "flare" by my RO. The difficulties have subsided recently after about eight months, and totally cutting out caffeine seems to have been the key to improvement. With only five treatments, the mapping needs to be excellent for cancer control and side effect minimization, and this depends on the skill of the RO you choose. If you're trying to minimize trips away from home, then you may want to ask the RO if an office near home could handle the fiducials insertion.
