Hello
I would like to ask
My husband diagnosed with prostate cancer Gleason
4/5 (2 cores) by Cedar Sinai, we sent the slides for second
Opinion for Doctor Epstein who said that
The results is 3/4 (2same cores)
Cedar Sinai pathologist said that
He believe That Doctor
Epstein missed few points in the biopsy slides.
Please tell me your opinion

Epstein is the Gold Standard. You can call Epstein's lab and ask for clarification if you wish. You can also ask the pathologist at Cedars to call Epstein to see if they can reach a consensus.
That's quite a difference. If it is GS 3+4, and I hope it is, all options are on the table: surgery, brachy monotherapy (seeds or temporary implants), or SBRT. If it is GS 4+5, there is one clearly superior option, in my opinion, and that would be brachy boost therapy with hormone therapy.

Smara, I was downgraded by Epstein on biopsy pathology from the local hospitals G8(3+5), to G6 (subsequently g7(3+4) on surgical pathology).
My surgeon, who is highly regarded, told me bank on Epstein, he is the gold standard. And MSKCC later reconfirmed it as G6

I would probably have more faith in Epstein's lab than Cedar's since all Epstein does is prostate cancer slides. The Cedar's lab has to look at all sorts of tissue. It's like having your Mercedes serviced by a Mercedes Benz specialist, as opposed to a shop that works on all vehicles.
But it's such a significant difference, I think you need to find out more. I very much like Tall Allen's idea of Epstein talking to the Cedar's pathologist and letting the medical pros work it out.

BTW, when Epstein reviewed my biopsy slides, I was (unfortunately) upgraded. So it's not like he always is rounding down. And most of the biopsy slides have the initial reading confirmed.
