Had Prostate Biopsy Wed--

OK..I had my prostate biopsy past Wed--(29 Nov )-

My cousin drove me & waited for me---& was there---when my Urologist came in-

My question--(& my cousin...noticed the same thing )

My urologist has a terrific reputation- & is a great guy--

When he came in-- before the procedure-- i mentioned my PSA had gone down (in 3 wks ) from 20 to
17. That's when he said.."Yes..and I'd be willing to bet- if we took another, it would be lower yet !"

(He is leaning towards some kinda inflammation )

THEN---After...the procedure--I asked him point blank..."Did you see the "suspicious lesions" that showed up...on the mp MRI....?"

His response was-- "Yes...saw them...2 very small, nothing impressive !"

My cousin...seems to think...(Like a Gastro Dr. ) he has seen ...hundreds...of lesions & can tell--with the naked eyes--something that is....worrisome...!

I kinda agree--(Then--near the end--he was talking of giving me Finasteride- to try to reduce Prostate size & lower..PSA )

OK....MY question-- Do you feel---an experienced Urologist --like my Dr--could just "intuitively" feel...when there is..Prostate c--or not...?

Could those 2 "very small" "Nothing impressive" lesions....could just be..."pockets of inflammation".. ?

I KNOW....we need to wait for biopsy results--just looking for some feedback..

I just felt..."Better" by the way (Tone) of his comments...

After I had my biopsy, the uro commented that there were some dark areas visible in the ultrasound, but they weren't do dark that they screamed "cancer" to him. So much for his guess -- I had PC in 70% of the prostate. I think they can make guesses but I wouldn't trust them a whole lot. You just need to wait and see. Best of luck.

Well- at least, by your answer, I assume Urologists CAN tell- when something DOES "Scream cancer" - during the biopsy- even tho your guy- guessed wrong.

Again- Thanks for the response. I knew there had to be one.

I don't have the answer, just my own extremely limited experience. From that I'd conclude that the uro thinks he can tell, but he can't, at least not with much confidence or accuracy. Most important, you just need to be patient and wait for the results of the test. That's the only thing that matters.

FC--Again, I was only asking if an experienced, good Urologist has an..idea...after giving a prostate biopsy-if there is pc or not. I did not expect a 100% accuracy---of course not !

You, unfortunately, were on the wrong end-of your urologist's educated guess

Just as with a DRE exam. Here, a competent Urologist can tell with say...85..or 90 accuracy--if there is pc or not ! There are times- as we all know--that men have gotten a "normal" DRE & later found--there is pc--(I knew a guy where this was the case---yet--they still give DRE --with a reasonable degree of accuracy ! Nothing...is 100%...-
Just that they are in..the small % of a false negative !

So--again-- in my question- I was not asking...(Of course not ) IF a Uro can tell..IF there is pc or not...
ONLY the pathologist knows for sure !! I was simply asking....IF..IF.....a Uro...has some idea...if he sees something particularly.."Worrisome"...or ..if things look....OK...!

So---FC---I again- make clear my question--Not if they can tell...after a pros biopsy...if he sees something that "cries" pc...-- only..IF he has some idea....--

I do hope I get others here who have Pros biopsies- where their Uro...has told them...anything--that either looked "worrisome"...OR--to the contrary...if things "Looked normal"..!

They (the experienced Uros ) must have seen---so many --they must have an "educated idea"....of what looks "Worrisome"..& what "Seems normal"....--after LOOKING at the "Suspicious lesions"...! Follow..?

Hoping I hear--from others on this---(Maybe Tall Allen...or someone w some knowledge )

One can think....or one can know.

Your urologist was thinking about whatever he was feeling or what is was seeing on the images.

The pathologist will know what he examines of the actual tissues.

The only way currently to confirm (know) there is PCa is by the microscopic examination at the cellular level by a specialist.

Educated guesses are not part of the standard of care in this case and most often useless. Most professionals use their extensive experience in discussing situations and setting expectations. My urologist told me I had cancer before the DRE or biopsy because of my PSA. I challenged this and he agreed that he could not confirm the statement scientifically, but in the many thousands of cases in his practice no one ever had the level of PSA without actually having cancer. But to know, he of course utilized the confirmatory process and tools of DRE for staging and biopsy to determine the PCa, extent, G score, etc. All of which need to be known.

glad your biopsy is over with. I asked the same question to the URO ...he said he saw a couple of black spots, but in my case he was quite sure I had PC although I had a 4.45/3.2 PSA because he could feel a firm and nodular prostrate doing the DRE - and he was right.

Just hold tight and you should be getting your results early next week. All the best. Hope you never join this club, but if you do its not the end of the world. Keep mr. happy busy next few weeks and you should see the interesting dark fall colors turn to winter white smile

YES--you mentioned EXACTLY what I (& my cousin )-were thinking !

These Urologists have done...literally hundreds--of prostate biopsies !

Even before the Path reports come in---I am sure they must have some "feel" with a 'reasonable' percent accuracy !

Heck..I know my Gastro Dr--told me he could tell a "Worrisome" polyp even before biopsy -

Mt Uro said right after the BX.............."with the size of your prostate that could be driving the PSA #'s" but of course a week later he told me I had PCa Gleason 7........Go figure.

Be well my friend and just deal with what you are dealt.
