Frequency, urgency, failure to completely empty, night visits ... seem to bother many of us. These symptoms are what first sent to to see my urologist. He prescribed Tamsulosin (generic Flomax). I had awful side effects: severe headache, sinus congestion worse than a bad cold, lower back aches, insomnia (at one point I only had 3 hours of sleep over 3 days & nights) then finally a horrible pain in the perineum area. I felt like I was sitting on a steel chain link fence post. My doctor changed to daily Cialis, which took an appeal to Medicare to get a "formulary exception" for its use. (Cialis is approved by the FDA for treatment of BPH, but insurance companies try to deny it because it is also for the treatment of ED which they do not cover). The Cialis has already worked to improve my urine flow and decrease urgency. After being on it for about 10 days with almost zero side effects (only a mild headache the first day). But, now I have a dull throbbing pain in my perineum. Do others have this pain? Is this common in the BPH and urinary treatment drugs?

I did not have any pain from Flomax or Rapaflo or Cialis taken when I had BPH.
Keep in mind that you have high volume, high grade prostate cancer (consider adding a signature, please) that is contributing to your urinary problems. The cancer may be blocking your urine flow. It is likely that once you start on ADT, that will clear up much of the urinary obstruction you are suffering flom.

I'm not a doctor Onthgodiy.....I only have my experience.
I had perineum pain for a long time before my diagnosis. I was also taking Flowmax for BPH. I can remember being at a basketball game (you know how hard those wood folding seats can be)....and moving constantly trying not to just get comfortable.....but....to ease the pain. To this day...I do not believe the Flomax (or my cancer) had anything to do with that pain. People with anxiety can have it more than others.....I seem to recall.
I spoke with a surgeon about this at John's Hopkins and it was his opinion that it had nothing to do with my cancer. To this day....I believe....it was pelvic floor pain.
My cousin loses work over it. So....this particular symptom isn't necessary indicative of disease or medication. It can.....just happen.
Not saying it is.....just sayin'
Haven't' had a chance to tell you I hate your diagnosis for you. But I do. You'll be around for along time.
