If you are only doing this to 40 yo men with a family history or perhaps African- Americans, that is fine. However, it is naive to think that other 40 yos won't over-react. The patient, in whom anxiety is a natural reaction, pushes his doctors. And his doctors, who wish to accommodate patients, complies. The recent 400,000 man UK study proved that population-based screening of 50-59 yo men is useless, let alone for younger men.

I read some of the good advice you got but to reiterate:

1) get your biopsy results checked. All your treatment choices hinge on the eyes and opinion of 1 pathologist. Johns Hopkins is reportedly the best.

2) if it’s consistent then why not consider an MRI to see if there are any other suspicious areas. If so you can get a fusion Biopsy where they use the MRI results for an ultrasound guided biopsy. No longer a random piercing of randomly selected less than 1% of the tissue. The tissue selected is based on suspicious observations. Lower chance they missed any diseased tissue.

3) If it is still determined you have G6 and only G6 then please consider Tall Allen’s initial response that appear to be a great candidate for AS, active surveillance.

Read all about treatment options and all about our screwed up healthcare system that gives doctors the incentive to recommend unnecessary treatments. Remember urologists are surgeons, believe in surgery, are committed to surgery and make most of their money from surgery. The doctors who would treat you with radiation are also urologists and get all of their referrals are from the surgeons and so are not incentivized to go against the refererers recommendation. Your urologist is a surgeon that can get $50 a quarter to monitor you on AS or thousands of $ to treat you with surgery or radiation. Try to read more alternative perspectives such as Invasion of the Prostate Snatchers.

Take your time, keep an open mind and get lots of different opinions.

I concur that you appear to be a very good candidate for AS. The JH second opinion is a must. I think you have time to think about it. If you can swing it financially, Oncotype DX or another genomic test can help with the decision making process. See if you can get your medical team to order a prostate MRI. Best of luck.
