What a crazy disease. First that it so directly attacks our very "maleness". Second these personally traumatic treatments we face (we don't use the term, but ADT is "chemical castration". Come on, that's actually a treatment? Really? I couldn't believe it at first, much less that it was for me). Third, the short and long term side effects that develop, the lifelong impact on us. And fourth, we get no sympathy for this whatsoever, and it isn't even something we can talk about.
No one cares about what is, for most men, one of the primary drives in our lives being completely devastated. "You should be happy you're alive". "At least you got the good cancer". Some old guy can't have sex anymore? "Good, they're just nasty anyway". Anyway... enough whining... it is what it is.
For what it's worth, I had that exact same side effect, just like you described it. Although, after a year on ADT Mr Happy had no activity whatsoever, and no interest in even trying. That part of life just stopped. When erections resumed after the end of 3 years on ADT, the physical change was strange. I can only assume the bottom 1/3 or so gets some of its blood supply, or maybe nerves, in a different way than the upper 2/3. It was clearly much different when "inflated".
Two years after the end of ADT, those changes have largely resolved and the little guy is actually very much like before all this happened. The occasional orgasms are almost completely dry., but still satisfying. To this day I don't have much libido even with a testosterone level higher than baseline; that is nothing like before. After this long that may never recover even though there is some physical ability. It's just not interesting. (Again, what a crazy disease!)
