Bad bladder control after prostate surgery

Had prostate surgery Dec 12 2017 still incontinence thinking of the Advance sling surgery but bladder just let's me use the bathroom a little at a time.
Question is this hoping someone has been through this procedure and can give me an out look the matter.

I considered a sling but decided my leaking was a bit more severe than a sling was likely to fix so I got an artificial sphincter (AUS) instead. I had radiation after my surgery and AUS's often work better than slings for men like me because the radiation makes the urethra less flexible and harder to reposition with a sling.

How many pads do you use a day? Do you use the kind that tucks into your tighty-whiteys or do you use pull-up incontinence underwear? Have you seen a physical therapist or biofeedback tech? Sometimes they can help.

Those guys will be along soon. It's a good thing to be considering at this point. We have a few who've gone down that path. Mostly good results. But like everything, it doesn't always work out as planned.

Gleason 6, had surgery, PSA now 7.0, is this correct ?

Yes Gleason score 6 and PSA was 7 and yes I was considering this sling since I didn't have a radiation and I use the pad that tucks. Thank you for your feedback Maybe the operation will get me to continuance best luck with your endeavors

Yes Peter thanks for the reply i use one pad all day at work can I change when I get home and take a shower I've done the Kindle exercises no therapy I think therapy probably be a waste of time as the urine sits all the way at the end of my penis and only thing stops it from coming out is the pad and to hold it way up the uretha have to dramatically get better so that's why I'm considering the sling to get out of these pads once and for all in urine I can get out the way of my 29 years of marriage sex life

You Better Know It:

Before you had your prostate removed you had two sphincters to hold the urine in. One was between your prostate and your bladder -- that one's gone -- and the other was downstream from your prostate. That second sphincter -- the one you have left -- is the one we can voluntarily control and after surgery we need to learn to keep it closed all the time and, often, we need to do exercises to make it strong enough. Since you are only using two pads a day, more or less, it sounds like your remaining urinary sphincter is almost working well wnough. If it wasn't working at all you'd be using the big pull-up diaper pants, maybe five or six a day. What a physical therapist could do for you is to make sure you are doing your Kegel exercises the right way, to see if you can't make that sphincter stronger. Often it helps and it seems worth a try to me.

What the sling does is to reposition your urethra in the area of your remaining sphincter so the muscle doesn't have to work as hard to hold it closed. It sounds like you would be a good candidate for a sling if physical therapy isn't quite enough.

I second what PDA says 100 percent. A physical therapist trained specifically in male pelvic rehab can do wonders.
Urine wicking from the urethra right after you're done peeing is, unfortunately, a persistent problem. I don't understand the mechanics of it, but it plagues me too, and I consider myself continent and don't use pads. Be patient at the urinal, shake it real good, and don't stuff your meat back in your pants right away like you did in your former life.

Question:: Is the PSA in your signature prior to the surgery? What is your PSA after the surgery? Your signature is confusing.
Answer to your question:: Have you gone to physical therapy specifically for incontinence. It really helped me. They trained my bladder to fill using a Cunningham clamp. They also had me on 'tens" therapy to strengthen my floor muscles.

PSA was A7 first check second check was A5 before surgery after surgery is a zero 0.10 .2 0.3 and so forth so Physical Therapy helped you did you have any other operation like the male sling AMS 800 you out of pads just with physical therapy

Had my exam today for the advance sling going forward with the procedure my urologist thinks it's a good move he said he cannot guarantee anything but I will be better asking all prayers thanks

Hello guys laying in the hospital bed thinking of you guys with the advance sling procedure done I had it done Tuesday was going home today but he had a hard time with the catheter getting past the sling so as a cautionary he wants me to stay one more day so I'll be going home hopefully cured Thanksgiving 2018 good luck guys and happy holidays
