Inter-operation Frozen Sections (IFS)

In the final interview process and ready to pick a surgeon. One mentioned he does IFS during robotic surgery to ensure he doesn't miss a margin and another experienced surgeon says he does not due to false positives and it is better to accept the risk. Both have done 2500+ robotic surgeries at major institutions. Do I weigh this into my selection? Most of the articles I've picked up would support IFS, but maybe I'm missing some nuance. If we are trading off risk, how do we play? Thoughts?

JNF's excellent suggestion aside, i'll address your question....

I vaguely recall reading studies showing that IFS is not proven to improve rates of recurrence, unfortuantely i dont keep these things on file to refer back to so i cannot say for sure. I'd also note that the studies were likely small scale as most surgeons do NOT do IFS.
With that said, i chose a surgeon who does Intra Operative pathology, and while it wasnt my primary reason for choosing him, it did give me some comfort. I actually had a positive IOM, which prompted further resection to obtain what was listed on the path report as negative FINAL margin. The surgeon, when i asked, stated that it was better than a positive final margin but not quite as good as a negative margin intr-operatively. So there's that.
I ultimately experienced a detectable PSA 2 years after treatment and underwent ADT + SRT which i did at 3.5 years out, when my PSA hit .08. I chose not to wait for the American Urological Associations official definition of BCR which is .2

So from my experience, i guess one could say that IFS is not the panacea that logic might make us think it is. But I'm just one guy.
As i said, not many surgeons do this practice, and it does make me wonder why, maybe those studies are correct. With that said, i see no downside, other than posibbly extending the surgery time by 15-30 minutes

Thanks for the comments. Yes, I anticipate that this will not be one and done. While the Memorial Sloan Kettering nomogram projects probability of cancer specific survival after surgery at 99% (10 yrs) the odds of progression free probability (biochemical failure) is 40-50% (depending on how you count the targeted vs random cores). Risk of extracapsular extension at time of treatment (now) is 57-62%.

It is my understanding that these numbers apply regardless if treatment is surgery or radiation. So I need to consider the total multi-year treatment path and pray.

One radiation oncologist said it would be ERBT + 2 yr ADT. The surgeon would go RP only and save the RT bullet.
New studies suggest ADT is linked to dementia (pops and grandma had Alzheimers).

Appreciate the thought on frozen section tissue collection during surgery. Think we don't have all the facts on this one.
