I'm so sorry you're going through this. As the others have indicated, it's important that you get treatment now as your PSA is already above 0.2. The ROs with whom we have spoken have all indicated that the sooner you treat, the better the outcomes. I don't mean to scare you but I do want you to understand the importance of acting quickly.
That being said, there may be time to get an advanced scan like the PSMA Pet scan. I know that you're at Kaiser, but you might try to get this a scan at UCSF. We were told that they're booking appointments at the end of January but, given your current PSA, they may be able to fit you in earlier. Kaiser may allow you to go outside its system and might cover this since Kaiser doesn't offer this scan--but I'm sure they'll make you jump through lots of hoops.
FWIW, two of the ROs mentioned a recent study that indicated that radiation of the prostate bed AND lymph nodes, plus hormone therapy, yielded the best outcomes.
