I am sending off the biopsy results to Johns Hopkins. Then I'm going to MD Anderson to meet with an expert in each area. It really sounds like they've made a lot of advances in radiation in the last ten years, so maybe that will be a consideration, whereas from the research I did 8 years ago, at my age then (42) I don't think I would have considered it.
Excellent plan. Something my urologist told me is to be prepared for surgeons to try to talk you into surgery, radiation oncologists to try to talk you into radiation, etc. I chose the robotic RP for the following reasons, which represents the underpinnings of my personal choice:
- My biopsy showed PCa in 12 out of 17 cores. While none of the cancer was greater than 3+4=7, the fact that it has spread in the general direction towards the perimeters is worrisome -- I want the beast removed.
- If I chose radiation, surgery would be out of the question in the future. However, if I chose surgery -- radiation remains an option later.
- In my case, I have a 57 percent chance that my PCa is organ-confined, and a 41 percent chance of extracapsular extension. That being said, my probability of survival after RP is 99 percent at both the 10 and 15 year marks. My odds of being progression-free after 5 years is 91 percent, and 85 percent after 10 years. To get these numbers, the doctor input my medical information into a Sloan Kettering Cancer Center prediction tool. This was extremely helpful in the decision-making process.
- I want to have the best chance of being disease-free -- and knowing it. With a RP, my PSA should crater to zero and stay there. With radiation, it's a little less definitive.
- For me, the alternative and experimental treatments (e.g. cryotherapy, etc.) would almost certainly have a less favorable outcome.
- My GP, who has no dog in this fight other than advocating for my health, rattled off a list of radiation side-effects (even with the newer systems) that -- for my age -- gave me pause. She was strongly in favor of the robotic RP.
As the saying goes, "your mileage may vary". The above represents my personal decision based on my own situation. I'm in the "unfavorable intermediate" category and aggressive treatment is a wiser choice.
