End of AS run?

Well I've be rolling along on AS for 5 years. My MRI results taken yesterday are on my portal. One (1) PiRad 4 lesion in Peripheral zone. No 3-5 lesions in Central zone. EPE, Neurovascular Bundles, Seminal Vesicles normal. I see my urologist in a few weeks. I expect a MRI fusion guided bx in the near future. I also expect treatment in my near future.

Your (potential) exit point for AS is my entry point to it. I wonder what my record would have looked like if I had had a PSA, MRI and Bx 5 years ago.

Anecdotally, there have been a lot of guys here with higher PIRADS scores that have still done okay upon biopsy of those lesion. Hope you can join them!

I have also decided it is time for me to leave AS. Your PSA is like mine. I have a yoyo, just with larger swings. The perinueral (SP) invasion was what pushed me over. URO said I could go on if I wanted, and 35% of his AS patients decide to opt out around the 5 year mark.

He said I would most likely need treatment sometime within 5-7 years if we let it ride.

My treatment will be in November.

It seems to me that having been able to avoid treatment for five years was a very good thing. Good luck with the biopsy. Never fun, but not horrible.

If nothing else…PCa offers far too many complicated choices. I have no regrets about choosing early aggressive treatment (SBRT) and avoiding additional biopsies, scans, mental stress and risk. Inserting the fiducial markers was easier than a biopsy and one planning visit followed by five easy and quick radiation sessions and my treatment was done.

All my systems work just as before treatment except reduced ejaculate. In my case…an easy choice. Good luck. Terry
