I see my RO in August and he'll do another PSA but I'll bring a copy of this one in case he's so interested.

Glen, actually I didn't have a spike that I knew of. My docs were so confident (or I was so trusting) that I did one PSA test at 6 months, one at 1 year, then annually after than. Had they been more often I might have seen a spike then a return to the downward trend. What I DID have was a long journey to my nadir. At 5 years out I was still at .2 and it was only in the 6th year and beyond that I was less than .1.

But to your point, there are lots of guys who do have spike and then keep going down. Maybe you will or maybe not. If you do, it may cause a bit of angst but rest assured that mostly it's a spike and that you are probably just fine. In any case right now you are lookin' good for sure.

Remember Glenn, you are slow walking a marathon. The slow downward trend is your friend and will likely have a few fluctuations along the way. Simply relax and trust the excellent treatment that millions have successfully used. Your early results are indicative of a long term success.

Glen - Sounds like you're right on track, my friend. You are exhibiting two signs of a good prognosis. First, a longer, slower downward PSA trajectory has been linked to better long-term outcomes among RT patients. Then second, RT patients who experience a PSA "bounce" also seem to do better in the long run as well. I think you're doing just fine.

In my own case, along with bouncing PSA, I am already 5 1/2 years out from my final SBRT treatment, and my most recent PSA is the lowest reading I've had in many years. I do not know if that is even my nadir yet (could still be headed down, or not). Only time will tell.

Any downward trend in PSA can really instill hope and lift a fellow up!

I remember once, my oncologist and I needed a round of celebratory handshakes and "high fives" when we watched my PSA creep down by just 0.01.

In that moment, after some tough treatments, it was the tiny SPARK of encouragement I needed to keep forging ahead.

I might add my lower urinary problem of irritation and mild pain when sitting is still here and even scarier, my hematuria is back these last couple of days
I wasn't sure about it being blood as I thought it may be dehydration but it's clearly blood and a lot of it.
Oncologist said it's a flare last time so I suppose it's still part of the healing.
Just the blood gets one's attention.
