Had my annual urologist/oncologist appointment this past Friday. We basically just reviewed latest uPSA results and where we were at in the battle. His thoughts on previous low uPSA results, dropping the coveted < , of 0.009 and 0.007, was in dealing with ultra sensitive test of no concern to him as merely "noise." That from his experience, I was still undetectable. Stated that based on time since both RALP (2017) and termination of SRT and ADT (2019), with undetectable PSA, he considered me "in the clear." Offered me the options, move to PCP for care now or continue with current testing (6 month) and annual visit with him. My decision was to have two more 6 month uPSA tracking labs with no scheduled visit with him. If anytime, I felt it necessary to see him to just make an appointment; otherwise, I am moving to PCP for annual PSA tracking after next two tests. The doctor recommended that following the next two tests that I be tracked utilizing the standard PSA assay, which I believe I will do.
So, following biopsy I was G9 with one core 5 + 4 highest. Opted for RALP with pathology downgrade to G7 4 + 3 with SVI and EPE and tertiary 5 less than 5 percent; therefore, pT3b. Undetectable PSA for one year, then SRT and ADT (6mo.). Since end of these treatments (05/19) undetectable.
The battle still continues as the foe is always in mind but there is hope. I still remember that "call" as though it was yesterday and the bottom falling out from under me. The key is hope with a positive outlook and finding a site such as this with the outstanding knowledge and support offered. You have had very good results for a G9 diagnosis. That’s nice to hear that you are taking a break other than the psa test. Congratulations.

I took largely the same path as you, although I didn't have ADT. about
five years after the end of RT I began to relax somewhat and went down to annual PSA tests.

I am about
where you are at. Last annual meeting (4 yr) with urologist was “you don’t need to see me anymore unless you want to … you can go to annual testing now or stay with 6 months testing if you want” which I expected at 5 years. I have been on “standard” testing (x.xx <0.10) since ART after surgery since there was no point in caring about
small numbers.
It is a shame that PCa dominates the mind to the degree it does with many men. The probability is pretty good that we will die from something else (if older) unlike many cancers where the cancer will be the cause of death in a much shorter time period. Time to move on to the life after cancer stage and not sweat the small stuff. Annual testing with a standard PSA test will let you know soon enough if Houston has a problem after the two part treatment scheme.
