Your TSH of 4.6 is high and indicative of hypothyroidism , also since your TPOab are positive this confirms that you have hashimoto's thyroiditis , which is the cause to your hypo.
Soon or late you should be put on Levothyroxine , to bring your TSH down to around 2.5 , you should insist on being put on brand name levo and not the generic form , of course if your insurance allow, this will keep your levels steady.
The levo should be taken first thing in the morning on an empty stomach one hour before eating or drinking any thing except water , and it should be take at the same amount each day.
