Diagnosed with UC a couple of years ago have been ok most of the time but for about the last 3 weeks have been going to the toilet many times a day feeling like im going to do alot but all im getting is blood and pale looking liquid although 2 days ago there was lumps in it very dark coloured is this normal for UC.

urgency is exteremely normal with UC with very little coming out at a time. were the dark colored lumps red? you may be bleeding more than you think. might be a good idea to have your GI check your blood counts. you may be in a little flare and steroid enemas work real well to help decrease the blood and urge to go. i think you need to scedule an appt to see for sure. your doc will prob need a stool sample to rule out c diff. that's prob not it because you would prob have much worse symptoms but worth it to check it out. good luck

cant remember if the lumps were red but they were definetly dark coloured also the water in the toilet looked almost black but that only happened the once the rest of the time the water is very red and the sides of the toilet are splattered red i also got a shock when i stood up and blood ran down the back of my leg. I do have an appointment with the IBD nurse and have already had blood sample taken by doctor so will wait to hear from them.

Yes, it's all normal for a UC flare.

What meds are you on at this time?

Asacol foam enema at the moment but i dont seem to be getting on so well with it i always feel really uncomfortable after using it and sometimes cant keep it in for more than a few moments. I had a colonoscopy last year and was told that aswell as inflamation low down there was inflamation along the transverse colon which i thought was strange as ive read UC isnt patchy.

If they are lumps of blood, then they are blood clots. I get tons of them when I flare.

kheal....UC isn't patchy...do you have a copy of the results from your c-scope?
If you have any inflammation higher, you should be on an oral med such as Asacol at least to start.

When's your appt with the IBD nurse? You need to write down a few questions to get definitive answers.

The symptoms you're experiencing are classic of an inflamed colon with the mucosal layer in a very fragile state. Sounds like CD in the colon (Crohn's colitis) but it's possible that it could be microscopic colitis.

You need to get a copy of the results of your scope.
And while you're at it...get a copy of the results of the blood tests too.

Sounds more like crohn's colitis, from my understanding microscopic colitis isn't patchy either.

yes....rectal inflammation can cause that as well...

but..are you eating enough food to digest?
what goes in is processed to come out.

You could be describing my current flare exactly. I started 9 mg/day of Entocort a little less than three weeks ago, and my symptoms have not cleared. If anything they are getting worse, and I am losing a lot of blood. My GI is not very accessable, so I'm going to start calling demanding an appointment. I'm thinking I need to move from Entocort, which until this time has always worked well for me, to Prednisone or something else.

I really hate Pred' side effects, but I'll put up with them if it'll get me back into remission. The last time I was on Pred' my job evals suffered from the mood swings.

I've been in a different job for the last few months with a different boss, so I need to sit her down and educate her about UC, and warn her about the Pred'. I'm not loooking forward to that!

wodin..you need to be on rectal meds as well...no need for pred as of yet in my opinion.

if you haven't tried them, do so....pred treatment for flares for your lifetime isn't a good plan.

My GI evaluated me for rectal meds during my last c'scope, and said that my UC extended too far into my colon for them to be effective.

I called for an appt. this morning, and he prescribed Pred' and a hematocrit over the phone, and said he want's to do another c'scope after I'm in remission again.

I agree with you and prednisone. I would gladly put up with the side effects to stop bleeding and the cramping, etc.

I think we think differently when over 60 - life is shorter (probably) for us and we want to enjoy all we can - as quickly as we can.

I am hoping if I need prednisone my doctor will ok it.  I might just try a short burst on my own.  Just got back from vacation when I kept everything under control with Imodium and pepto bismal. 
Waiting to get back to normal but so far feeling pretty good with NO meds just some supplements.

Well I picked up the script for pred' about an hour ago, he's giving me 60 MG/day. I guess he doesn't want to fool around. I'll have to monitor my mood swings closely and take sick leave if necessary. Wouldn't want to get fired for popping off at the boss.

The blood tests came back just a few minutes ago as follows:

wodin...just for the record....rectal meds aren't meant to treat beyond the sigmoid...that's what the oral topical meds (asacol and entocort that you're on) are for.

Yes, that's what my GI said, and he said that much of my colon above the sigmoid was involved, therefore rectal meds would only be a little efficacious, and it was better to treat the whole colon with oral meds. There might be other opinions, but he's my GI and I'll follow his prescriptions until he's demonstrated himself to be incompetent.

You should have still given them a fair chance.....

I'm not saying your doc is incompetent...I'm saying he's remiss in not having you try them initially before the prednisone.

The asacol and the entocort should be enough to treat above the sigmoid....meaning however much is involved..that in conjunction with the rectal meds.

I know from experience that oral meds such as asacol CAN make symptoms worse.  I had to discontinue all mesalamines, etc. and go to prednisone and 6 MP because all oral meds I tried made me worse - more bleeding, cramping, etc. Good luck with the prednisone - hope it works for you.

wodin, I have pancolitis, and my GI said it was a good idea for me to be on mesalamine enemas as well. He didn't prescribe them initially (I asked about them at my 2nd post-diagnosis appt, which was a week after my 1st post-diagnosis appt), and he'd said he hadn't prescribed them initially b/c my symptoms were so severe he didn't think I'd be able to retain them at all, and wanted to see what Asacol alone would do. The Asacol had started to help a tiny bit, but when I added the enemas, my symptoms got a WHOLE lot better right away. The topical rectal meds (enemas, suppositories) have been studied extensively, and are MUCH more effective at calming inflammation.

I really don't agree with your GI. Yes, the won't do anything for the inflammation that's higher up in the colon. But they WILL do so, so, so much to heal the lower part of your colon that they DO reach (and that oral meds can't always reach). I completely agree with quincy that your doctor should have prescribed enemas before prednisone. My doctor HATED that he had to prescribe prednisone to get me out of the flare. But, I had an interview coming up, it had been over a month and a half since my diagnosis, I had been ill for four months, and I had a 5 month old at home.

Well, I took 60 MG of predisone 6 hours ago, and already I'm seeing some relief. It feels like something is actually moving now instead of just teaspoons full of bloody sludge. I had my first semi formed stool in a couple of weeks a few minutes ago, and while there is still a lot of blood, it seems to have diminished some.

It really sux that prednisone works so well and has such awful side effects!

Have been prescribed with Asacol tablets and supposetries the IBD nurse also told me that according to my notes i have what they call indeterminate inflamatory bowel disease she also told me my last blood test showed that the inflamation was very active

Sounds like you are on the right track - treating from both ends.  Hope it all works for you!!

kheal...if you don't show any improvement over the next week or so, call back and request the retention enemas. But, in saying that, you could opt for using the suppositories at night and in the morning.

Then if no improvement, ask for the enemas.
