I have BCBS AZ. It is a very large insurer in the state and has many group and individual plans. What doctors are in your plan will vary. Mayo in PHX is not in my plans PPO but is in some other plans. You can use their website once you get policy info. I have not run into many hospitals or doctors that are not in my PPO. You can even check costs for drugs and common procedures. Costs of colonscopies vary tons between cities and doctors.

Now for the bad and good news.
Bad. You really need to get precertifications, get tests sent to independent labs that are in PPO, and check to make sure every doctor and provider involved is in your network. Otherwise you will find you receive surprise bills. Especially if you are not in PHX, you will find long waits for appointments and some really bad doctors and hospitals. I have yet to find a GI or primary care doctor that I wanted to return to. GI's are either in independent outpatient clinics(colonscopy mills) or are affiliated with major medical centers(long waits and expensive). Both have negative features. Providers and BCBS are more than happy to put you in the middle and bully you around.
Good. BCBS is better than most insurance companies in the state. Tucson and PHX have CCFA support groups that probably could help finding a doctor.
