Question about bleeding with UC

Hi everyone,

I am (as yet) undiagnosed UC, and waiting for a c-scope for diganosis. I have had rectal bleeding with some pus and diarrhea for ~10 weeks and tried a variety of meds.

Currently I am on 4g pentasa (oral)/day and a 1g pentasa enema / day (for the last two days).

My symptoms in general have been decreasing, but since Friday I have had more bleeding than ever :-( I have been stressed about not having a diagnosis etc. so wonder if this is contributing...

Anyway, my specific question is: My blood all comes out before any stool (and there are two kinds, lots of bright red blood and some pink mixed with pus). Is this consistent with bleeding being in my rectum/sigmoid colon? i.e. if it was higher up does the blood work its way down?

I am away from home at the moment, so will make appointment to see GP shortly. But I was wondering about other (more experienced) peoples experiences of bleeding.

My blood seemed the same whether I had UC proctitis or pancolitis. I had the bright red also and pink mixed with mucous

never heard of a 1g Pentasa enema...do you mean suppositories?

If your blood comes out first with mucous or whatever else...it would probably mean it's from the rectum and possibly the sigmoid..depending on how much you have. Is it with diarrhea?

Are you having more formed stool as well as the bleeding and mucous?

The lining bleeds easily after a bm or because it's friable or fragile enough to bleed on its own. Whatever is in the rectum will come out..whether it's blood, mucous, gas or stool...to be followed by what's in the sigmoid.

It's probably mucous and not pus.

If you're on the suppositories, they can be used up to 3 times a day....maybe call the doctor and see if you can get the retention enemas..or at the very least to use the suppositories twice a day.

The enemas would be better.

I've got pancolitis.

When whole colon flares up, the colors of blood are from bright red to dark, almost black.

Now that I've got only a couple of inches of rectum inflamed, the color was bright red and came out first as you describe.

I guess the docs are unable to come up with correct diagnose. No unusual. They can hardly separate Crohn's and UC.

You definitely need to be seen by your GI, and the sooner the better.

Hopefully once you have a full work-up, they'll tell you it's just hemmoroids - that would be good news.

Thanks for your replies.

Quincy - I am on 1g Pentasa/100ml enemas - I only took them twice though as I was worried it was making the bleeding worse. I will start again when I return home tomorrow. Still not having formed stools - unfortunately.

KiWi44 - thanks - this gives me hope that actually the colon is responding to the oral pentasa, and hopefully the rest should follow suit when I start the enemas properly. I had been using asacol supposititories (so much easier than the enema), but I gather from my reading on here that the enemas are better at getting higher up, so will give them another go! BTW - your username makes me ask if you are from NZ? if so me too!

Dsm4life- I know it seems crazy that I am 'undiagnosed'. My GP suspects UC based on a physical exam that I had 10 weeks ago (he saw an area of ulceration and inflammation in an unprepared sigmoidoscopy). However, I only got to see a GI last week, and he was really cagy about what he thought. IN fact he sent me into a complete tail spin as he mentioned the "C" word. I am scheduled to have a colonoscopy within the next few weeks which will (hopefully) give a definitive diagnosis.

Hi...ah, NZ. I did the research to see that the 1g is available, if you can eventually get on a higher dosage, that would be better.

Good to hear that something works. Yup, suppositories are for the first few inches and enemas affect higher. My last colonoscopy showed only two inches of rectum being inflamed (the best in 25 years) so Canasa sups it is, and already the minuscule symptoms have disappeared.

I forgot to add last time: My number one trigger for a flare is stress.

Let me put it this way, I can move to NZ, which I love, if the need be. (It's a long story.)
