Asocol HD - pull me out of a flare?

Having a moderate flare - last time around I ended up on pred because I let it go too long before going to see my GI

PA just put me on 2400 mg of Asocol HD (had been on Delzicol)

Will this pull me out of a flare?

It's not likely to do it all by itself without help of some kind. What are your symptoms?

I got out of a flare up once, when I was first diagnosed, by using asacol 3600mg. I continued my rectal meds too of course. In my case, I was not on oral medication at all prior to that remission.

current symptoms:

Urgency in AM prior to BM
stool mostly formed with signs of blood
initial BM I am seeing some minor clotting in the toilet, pink on TP
2-3 BM's in the AM, nothing further throughout the day
intermittent foul gas
no cramping

I had been on hydrocortisone enemas for a 2 week period, 4 weeks on Mesalamine enemas prior to the hydrocortisone.

Strangely enough, the PA did NOT renew my enema Rx.

I think there's a chance the oral meds will pull you out of the mild flare up. You may need to go up to the max dose but you migth as well see how you do on what they prescribed. I would definitely request a refill of the enemas and possibly get suppositories for using during the day.

I was thinking same about the enemas. In your opinion, which would be more effective, the Mesalamine or the hydrocortisone?

The MD was out of the office and I met with a PA so I will call and talk with the MD on Monday.

I was also under the impression the max dose for Asocol HD was 4800 mg/day but I'm only on 2400.

I came out of two early flares after I was first diagnosed by just having my asacol increased. I haven't had any problems with flares in almost ten years until now. I was just put on Prednisone and I'm not thrilled about it, but it was either that or the doctor wanted to put me on Imuran.

I think it would be good if you could get things going in the right direction with the mesalamine enemas but I wouldn't rule out cortisone enemas if you're not noticing improvement. I would also go up to a higher dose of mesalamine if you're not noticing a difference but hopefully you do well on 2400mg and you get into remission and stay there. I think 2400mg is often seen as a maintenance dose and higher is for fighting a flare up. Since things sounds somewhat mild in your case, 2400mg may work out. It's all a lot of trail and error with this illness.
