Hy..few days back i experienced the worst symptom of this Disease ...TENESMUS.. My body was exerting so much force ..i wasnt abl to come out of washroom even for second.i literally died that day..my Gi prescribed me antibiotics for 5 days and i did well..but now after 15 days im again experiencing same thing, im nt able to contact my Gi this time..should i again take same antibiotics....im crying plz help me

Tenesmus is the feeling that you have to poop but no poo comes out.

If you are having this uncomfortable feeling I suggest some pain killers. When my body tries to push but nothing is there to breath deep and try to calm your body from trying to force nothing out. This is very hard to do.

If needed I suggest a cpl tylenol PM and try to sleep until you get a hold of your GI tomorrow.

Yes u r right im experiencing same symptoms. I took pain killer bt nothing is helping. Should i take antibiotics which my Gi recomended last tym when i had similair symptoms?

I have no idea why your gi would have given you antibiotics last time for this. And most UC patients find worsening of symptoms when they take antibiotics.

I suggest a warm bath to ease and relax the muscles.

Because he said it might be due to local infection..so should i wait for the symptoms to disappear on its own..im tryng to keep myself hydrated..its so hard ..

I would not suggest taking the anti-biotics. The only thing you can do when your bowels and muscles are all spazzing like that, is to try to relax them.

I suggest getting into a warm bath. Even a hot bath. That will physically relax your muscles and ease your pain.

Try breathing exercises. Big deep breaths and think of something happy that you love. Focus on that and breathing, not pain.

If your only problem right now is the muscle spasms and the bowels trying to poop out nothing... a bath is about the best bet to help asap. It should be pretty immediate.

If your anus hurts at all, maybe put some Vaseline on it when you get out of the bath. Call your GI in the morning or talk to a GI nurse and see what they think.

It is just your flare, nothing else. I feel that way ALL THE TIME, when I am at peak of my flare. I would recommend, take Bentyl 20mg and Acetaminophen (Paracetamol) 325mg or 500mg, depending on whatever is available. Take this combination 3X a day and it will give you some relief.

Bentyl will slow down your very active colon and tylenol will take care of cramping, along with Bentyl. Talk to your GI about mesalamine and hydrocortisone enema. In theory, 40mg Pred should work just as well, but we don't know much info about you, so can not comment more. How long are you on 40mg Pred? How much do you weight? 100mg Imuran may not be enough if you weigh more than 100lbs/50kg.

I don't understand, why your GI would prescribe antibiotic for this, unless you have some infection showed up in the stools test or something.

What probiotic are you using?
Have you thought of L-glutamine, slippery elm supplement?
Do not use fiber supplement.
If your rear end is irritating, use Xylocaine 5% gel, it will numb the end.

Are you on any rectal meds? Ive heard they help alot (although they did not help me personally).

@deltaforce:: even took acetaminophen 325mg..im feeling much better..im on 15mg pred (tapering)..i use mesalamine suppository..my weight is more than 50 ,since im not bleeding so according To my Gi i dnt need to increase my azoran dosage(dnt know he z right or not). I take saccaromycis bollaridi 250mg as probiotic..and abt slippery elm, i would try that..thanku..lots of love and hugs.
@conquer uc:: i use mesacol suppository..and my Gi adviced me to sit in salt water whenever i feel rectal pain, thanku ur reply means a lot ..

Yea - dreaded tenesmus is pretty bad.
I used to get it prior to UC days, and the only thing that got it under control was a homeopathic remedy called: Mercurious Sol.

I used to carry it around with me all the time, just in case it happened.

Within a few minutes of taking the remedy, it stopped.

As you may be aware, homeopathic remedies have no side effects.

What do you think about getting this remedy to have on hand?

And that would be true why, exactly?

OP, tenesmus sucks, and antibiotics relieved it for me too, so don't feel weird about that. As a short term solution steroid rectal meds would probably help the most. Sounds like you're outside the US so I'm not sure what the brand names would be, but here we have steroid foam enemas that are really easy to retain and can improve the symptoms quickly.

homeopathic remedies absolutely can have side effects, so that's a bull statement.

Call your doctor and request stool samples to be done.

I'd suggest another probiotic other than just the boulardi.

Definitely ask for an antispasmodic. Dicyclomine works well for me.

Did you recently taper the prednisone?

Are you using the rectal suppositories every day?

The only thing that aids truly in clearing tenesmus up is using rectal meds, it's basically an indication that there's inflammation in the rectum, therefore you need rectal meds to clear it up, the longer you wait to get on rectal meds (cortifoam is the only rectal med that works for me) the longer it will take to clear it up.

You can still have tons of urgency while experiencing tenesmus, it's just that even after you're finished "going" on the toilet it still feels like yo have to go because of that specific pressure feeling you normally get when you do have to "go" doesn't ever go away....that's rectal inflammation (aka; proctitis).

I am baffled. Are you in the US? You have pancolitis and your GI gives you a suppository? I think you would need enema, it goes little more in than the suppository.

And taper VERY SLOWLY, 2.5mg every 2 weeks if you can. This is little critical phase. I tapered down 2.5mg every 4 weeks from 20 to 5mg.


Your post certainly pulls some nerves..
