short term type-II is usually reversible because it is mostly about higher sugars (like form steroids) and greater insulin resistance (like from weight gain and inactivity). if you get off steroids and lose a little weight and get some exercise, it is a good chance you can stop insulin. You may have to continue a drug like glcuaphage if your colon can tolerate it. if you can't tolerate glucophage, there is some chance you might need a low dose of 24h slow-acting insulin - but you should be able to avoid fast insulin with each meal.
if the condition goes on for too long, the pancreas will work overtime to produce insulin to counter the insulin resistance of your cells. Like running a car in low gear, this will eventually kill off some pancreas cells. Once 50% of your insulin producing cells are gone, you are forever diabetic, even if you can get rid of the insulin resistance - because 50% pancreas cell death is definition of full-blow type-2 diabetes. . Still, with diet and exercise, you may be able to make out OK with half a pancreas.
