Blood went from red to black purple

I've been on my third day of enema cortison. My blood is no longer bright red but rather darkish like black purple. I thing It because the blood is reducing and I'm only expelling the old left over blood.

My bowel and rectum are really sensitive, for instance, I can't side on a chair to quickly because de pain will irradiate from to the anus to my left side and transversal colon.

Yes, it's blood mixed with the enema. If you aren't going often, the blood will pool or sit in the area from your last bm.

It's normal to have that during healing and using the enemas. As you improve, you might notice more mucus as well....

Can you take any 5ASA enemas?

How long is your enema prescription for?

I remember mine turned dark purple right before the bleeding stopped.

I always get the purple stains in my toilet when I am using a lot of enemas. I hope they are helping you heal :)

Yes, I had dark purple a couple of times when using cort enemas.

Purple or dark blood is from higher up, not close like from the rectum.

While that's a common "rule of thumb"...not when mixed with an enema or has pooled in the rectum for a long while between bms.
It can also be a tad darker or purplish/maroonish when mixed with mucus .... in the rectum....again, when sitting there for a while..

I base that on my experience....with enemas and without. Never had any bleeding elsewhere other than the rectum and into the sigmoid.

Yes I take 5ASA, Salofalk enema and suppository at night and cortisone enema in the morning. I have 3 weeks of cortisone enemas left. And I plan on tapering down this week.

They were prescribed to me in January by my family doctor but I did not use all the refills. When my GI she saw me a week later inflammation and bleeding had reduced.

No oral med since inflammation is in rectum and distal in the sigmoid. GI says, it would not have any effect to take oral meds.

I always see my family doctor first when I'm not doing well since she sees me faster.

My GI is on vacation for the month of July and the blood and frequencies were increasing. I had tried 2 Salofalk enemas a day but it did't help. This is why I've tried the cortisone enema.

I was worried that the purple blood was coming from higher up, as IPoo said because my colon really was more painful than usually.

The blood pooling does make sense as I alternate between D and constipation. I either have the D or the constipation with few little tiny poo. The pain has decreased since yesterday. It must be a good sign.

You mention diarrhea, which means it's high enough to cause it.

I disagree with your doc to not have you on any oral 5ASA since yours isn't limited to ONLY rectum. Even that, being on a very low oral dosage is helpful in possible spreading as well as it's believed that 5ASA is helpful in preventing colon cancer. All plusses.

It's not the fact that it won't help your rectum...it's the fact that you HAVE UC, and your GI cannot promise you that it won't spread. I've seen quite a few with strictly rectal meds, only to have symptoms go beyond the rectal med reach.
Just saying....

Since you're taking Salofalk enemas (are you in Canada?)....generic asacol is available at 400mg each.

I have actually been thinking about switching GI. She seems to be the only one not worried about the UC spreading more or having relapses.

She always got me to get of meds when I'm symptoms free. I asked why and she says my case is too mild to think about preventing relapses.... But from what I have been reading it does not seems to be the general approached by GI.

I have been in this flare since I've stopped smoking. Maybe, if I had also been on oral 5ASA, I would have pull out of this flare these last tree times I thought I was over with it.

very weird she'd take you OFF meds when you're doing well. Not the usual approach for UC....mild or not.

The fact that you quit smoking would probably have a hand in what's going on. If you were smoking during diagnosis and just quit now....you have no idea the actual extent of it.

She's playing an unfair game and you're the pawn.

If it was hurting you to sit in a chair then the inflammation must have gotten worse, unless butt irritation or hemmies made you so uncomfortable. When you use cortenemas, I would recommend that you take them at bedtime & try to retain overnight. Any med you use in the morning is likely to come out sooner-- so not as helpful. I agree with Quincy that you need doctors who take the possibility of flaring & extension of UC to upper colon seriously. A practitioner who advises you to quit meds when your UC seems to improve probably does NOT have enough knowledge about our ailment or experience treating IBD patients longterm. / Old Hat (33 yrs with left-sided UC; presently in remission taking brandname Colazal)
