Flex sigmoidoscopy results and Entyvio

Hi everyone. I just had Flex Sig today morning and results show "Extensive, confluent ulcers with erythematous edematous mucosa throughout rectum. Formed srool with normal mucosa begins at splenic flexture."

My doc told me that there are still ulcers present on descending colon and in rectum area but transverse colon seem fine. And that goes with my formed stool presentation. I am taking humira once a week and that has helped me get my stool back to normal shape but I still have little bleeding after passing formed stool and blood on tissue. I go to bathroom around 2-3 times a night and 2-3 times a day with just passing mucus during day and formed stool during night. My GI increased Hydrocortisone suppossitories to twice a day. He ordered blood test and said he would like me to start on Entyvio. But since i am having formed stool i do no want to d/c humira nd start new med. what if it dsnt work and I start flaring, having diarrhea. Any suggestion?

Something seems amiss with what you stated and your symptoms. Your symptoms match "Extensive, confluent ulcers with erythematous edematous mucosa throughout rectum".

They don't match "ulcers present on descending colon", unless he means individual ulcers and not the continuous inflammation ulcerations. How far up the descending colon does he mean?

I don't know much about using Humira or how long you've been using it, but it does seem it's following the usual pattern of healing with quite a bit to go yet.

Once your rectum heals, you should be heading into a better state regarding your colon starting to function more normally.

Can you not use mesalamine? If you can, start the enemas in conjunction with the steroid suppositories.

Are you on oral mesalamine as well...or just the Humira?

Are you on any fibre supplements?

Are you using any antispasmodics?

Good to hear you're improving. I agree with pb4 to stay on Humira for a while longer to see if things improve. If things don't...then to switch.

Regarding the sigmoid (was that what the doc considered the descending?)....do you have info on that as well?

You might consider to ask for steroid enemas at night and enemas during the day. I'm suggesting that for it to get up and into the sigmoid to help heal the area.
The supps won't be of help with that.

Cortifoam is a rectal med, it's a foam med not liquid and it can absorb much better than suppositories, I've always had success using it just before bed....it's a steroid based rectal med so it in my opinion works better (based on my experience it's the only rectal med that has helped me). I was instructed to use it twice a day but was so bad off I could only use it before bed in order to retain it....it doesn't have the side effects like oral prednisone because it doesn't get into the blood stream the way oral pred does, though some say they are quite sensitive to steroids and have experienced some side effects but nothing compared to oral pred.
