My last two BMs have been bloody, which is virtually unknown for me. The first time I thought - apologies for TMI in advance - my period had started, it was just pure blood on the toilet paper.

I don't really understand the significance of bleeding. Does it only occur in the presence of severe rectal disease, or can you have it with milder disease? I had severe Crohn's colitis for 13 years with no bleeding (only tiny specks of blood on the paper occasionally), but that may have been because my rectum was largely spared. Since the reversal, my Crohn's has taken up residence in my rectum. It was mild a few weeks ago and presumably still is, but would increased bleeding mean it's getting worse?

Or maybe I just have a hemmie... :-/

Well UC and blood go hand in hand for me. Never had a flare that I didn't bleed. I've had blood with mild, moderate and severe flares. So, for me, blood doesn't necessarily signify that I'm in a severe flare, only that my flare is getting worse. Meaning that I have a progression with my flares. First comes the increased gas, bloating, spasms/cramping and then usually the mucous and urgency, then the blood and then the bloody diarrhea. These symptoms pretty much always happen whether I'm mildly or severely inflamed. The difference for me between a mild, moderate or severe flare is how many bowel movements I'm having and how much pain I'm in.

You may be getting it now because of the rectal disease. I thought I remember reading that bleeding isn't as common in crohns as it is in UC when the crohns is affecting the small intestine/terminal ileum. But it's pretty common in UC. I know you have crohns colitis, but if I recall you had it mostly in the terminal ileum/ right side correct? I think you may be bleeding because you are having rectal inflammation now that you didn't have before.

BTW, how did your MRI go?

Blood is my most frequent symptom, and I have had it when there has been any kind of inflammation at all. It was my first symptom when I got diagnosed with UP. So perhaps, as RockerGirl said, it's related to the issues in the rectum.

Or it could be a hemmie.....

Blood can slowly seep and pool over time from inflammation and ulcers. Mild inflammation can result in passing a little blood with just your first bm of the day, but not subsequent bms (it pooled for hours while you were asleep and your bowels less active). More moderate/severe inflammation would be with every bm.

I should add I've had rectal inflammation for 6 months with virtually no bleeding. But I'm not sure if that's because it's mild or because of something else.

I had pancolitis really, but it was worse on the right and in the transverse than on the left side. Also had terminal ileum disease as well.

I dunno how the MRI went yet. It takes about 2 weeks for the results to come through. I'll give it another week then I'll ask my IBD nurse for the results (she's the only one I can get directly in contact with).

Thanks Rockergirl :p I'm gonna be off to bed after this post. Got a psychological assessment tomorrow that I'm rather dreading... :/

Edit: Oops. Thanks songlady and iPoop. My two bloody BMs were in the evening, so the blood wasn't from pooling overnight. That said, the blood seems to have passed now. I dunno, it's still a mystery to me why I don't pass blood or very rarely. I'm not even sure how mild the inflammation really is, as my docs have contradicted themselves a bit on that one.

you still have your rectum, and it could be that you have inflammation that's progressed into it being friable...more easily bleeding as to its fragility.


Was the blood in the bowl as well as on the tp?

It could be that your period hormones have kicked up the inflammation a notch.

If it's up and down, you might have a fissure?

Have you had any rectal meds in your regimen since the inflammation has started there?

My internal hem's used to bleed like crazy way before UC. Especially after drinking alcohol. Pure blood in the toilet and on the paper. Scary as hell at the time. For me, with UC, the blood was not significant. Just my experience.

quincy - yeah, the water looked red.

If I have a fissure, then it's a painless one :-/ That said, I never have any solid BMs to aggravate a fissure, I suppose...

I did 20mg prednisolone foam for about 10 days. It didn't seem to really make any difference. I'm considering asking for a course of oral steroids, partly as a test: if 40mg pred nails the symptoms on the head, then I know it's my Crohn's and not, say, IBS causing them. But I think I'll wait for the results of the MRI first.

Uniform Charlie - I forgot to ask both my surgeon and GI if the flex sig showed any internal hemmies. So I still don't know whether I have hemmies or not.

I would have blood literally dripping out of me when my hemmies (both internal and external) were flaring up. That's not to say I wouldn't get blood from a colitis flare but it was never as bad. Mucus was a different story.

This past flare i had the most blood where i was almost admitted to the hospital. I would pass no stool. Just blood. I have been flarimg since last march
