Inflammation thickens the walls of the intestine, reducing the space available for stool. As a result, stool is often thin, pencil-like, and often has a flatspot/groove on one side. As you heal, stool gets thicker.
Some do have a short response to remicade during the loading doses. The purpose of the loading doses is to get remicade up to a therapeutic level (optimum concentration to heal you) within your body. If it doesn't last long enough after the initial loading doses then you might need a stronger dose, like 10mgs per kilogram instead of 5. Or more frequent infusions.
