Entyvio Not Working?

I've been on Entyvio for over a year now. I did my loading doses in January of 205, and while things got slowly better for the first few treatments, I've since had no progression. I still have urgency/blood/mucous when I make my dozen or more trips to the restroom everyday.

I've tried Remicade, was on that for over a year, with no relief.

I'm currently taking Rowasa every night, but that's the only daily maintenance I'm on.

What are the remaining options for someone who's tried Remicade and Entyvio and has had no success with either?

Hmm that's a tough spot to be in as certainly you've tried the big guns of entyvio and remicade. There's always things you can try, your symptoms are real bad which is always a consideration.

What's your entvyio dose? Some gastroenterologists are increasing that dose.

You didn't mention immunomodulators (like Imuran, 6mp) which are often combined with biologics like entyvio. Beyond that methotrexate or cyclosporine are stronger options.

Other biologics approved for uc are humira and simponi. Some are taking stelara or xeljanz off-label. Clinical trials are available for those so inclined.

Have you considered a surgery consultation? In your situation with your symptoms I'd at least begin exploring that possibility as some just don't respond to any uc medication.

It sounds like the Entyvio is not working. My husband had 6 or 7 infusions, including towards the end shortening the time period between the infusions, but it really did not seem to help. You should NOT be having the symptoms you are having. Husband could not get off of prednisone, and he and doc concluded the Entyvio just wasn't working. Only thing that worked consistently was steroids, but that is not a long-term option (already been on them for almost two years straight).

At that point, we started to explore surgery options, just to be smart about it and start getting psychologically prepared.

At the last minute, our insurance approved Xeljanz off-label and we are in the early stages of seeing whether that works.

Clinical trials for UC using Stelara have just started, so that may be something to explore. The problem there is you may end up on a placebo, although my understanding is that will be evident early on if symptoms don't improve and then they will put you on a drug.

I tried Entyvio for 7 months. It didn't work for me but it cleared up my cystic acne. Once I stopped it, my acne came back. Go figure.


Plenty of other biologics or of course IVIG.

If it is just your rectum that is bothering you then maybe steroid enemas might work for you, it may be just the push that your UC needs to get into Remission with the Entyvio. They have less systemic effects then Prednisone.
