Hi
Since my last colonoscopy about
a month and a half back which showed everything healed except mild erythemea in mid sigmoid colon have been having blood streaked stools on and off inspite of doing mesalmine enemas nightly and 4.8 grams mesalmine orally . Got a sigmoidoscopy today and doctor said there was just the one small patch in sigmoid with mild activity but did not show any signs it could be bleeding . So where is the blood coming from . I am at my wits ends because now it seems to be coming and going . Tried doing the enemas twice a day for a month but this is the only symptom that persists . There is no urgency , frequency is my normal 2-4 bowel movements a day depending on how much I eat . Gi said to forget about
the blood and it will settle down . Any ideas on how to proceed because no one is giving me any answers . The enemas definitely help because he said that the rectum looks great .

Hi...how often is on and off?
Are you on probiotics?
Did the doc take a biopsy of that ulcer and surrounding area?
Have you tried alternating with steroid foam enemas?

Was waiting to hear from you 😊 Biopsy was taken when colonoscopy was done in July . Report said quiescent ulcerative colitis with mild activity . But both times there was no fri ability seen so doctor insisted could not be bleeding . No biopsy was taken today . On and off is its there in the morning stool but then won't be there in the rest . Or it won't be there for two days . It's all very strange . Steroid foam is available here ,not enemas . Not sure if it will reach the mid sigmoid. I am on vsl 3 probiotics

Should I start the steroid foam which is known as colifoam there . Doc said its not required

Steroid foam one night and mesalamine the next...or steroid on morning and mesalamine at night...yes, the colifoam will reach the sigmoid....proctofoam wont.
So blood with only one bm? What do you mean by the rest...how many times a day and when do you go?

Could the blood be from straining during your bm? Could it be from the enema tip (sometimes a jagged edge or insufficient petroleum jelly might cause a little bleeding)? Any history of hemorrhoids as they bleed as well?

This sounds like a hemorrhoid or small wound in the rectum that gets
opened with the stool. Maybe more with morning stool because it sits there longer.
Usually IBD bleeding has some level of mucus involvement.
I am not sure how far into the rectum scoping really starts. I always assumed that in initial inch or so was missed.

Definitely could be....one would figure the doc would at least investigate that area hoping for a simple explanation....
Meaning, its an obvious first for any bleeding.

I poop normally three to four times a day , sometimes the blood is there sometimes not . He did say I had a small hemmi so maybe that's getting irritated with the tip or something . Will start the steroid foam in the morn , how many days should I do the foam

He did a very thorough scoping , I thought it would never end , I was without any sedation and saw the full show , he is adamant the bleeding is not colitis related but a cut or something of that sort

Quincy the steroid foam reaches 35 CMs up which is where the patch is ?

Apparently it will reach into the descending colon... how far up I have no idea. Absolutely for sure it will get into and past the sigmoid.
