Lialda Pseudo Colitis or trigger for actual flare?

Hi Everyone,

I have had UC for 10 years and for the most part am able to manage my symptoms with diet, etc....which is good because I fall into the small category of people who have an intermittent intolerance to Asacol, Lialda and I'must assuming all of the NSAIDS used to treat IBD. My GI has had me on a low maintenance dose of lialda, but I had not been taking it regularly while in remission because of the adverse affects it can have on other parts of the body.

So....last Saturday I noticed the dreaded white spiderwebby substance on my stool indicating that at the very least, some underlying irritation had occurred. I took a dose of lialda....the following morning there was blood and mucus in my stool. I haveven not taken another dose of lialda, and while my symptoms have not necessarily gotten worse, there is still blood attached to my stool. The frequency hasn't changed, as I usually have 1 to 2 BM's in the morning.

My question to those of you who have had adverse reactions to lialda is whether or not the irritation persisted over a period of days, how severe was it, whether or not it was accompanied by blood/mucus and did it self resolve or was a steroid or some other medical intervention necessary?

If you do have an intolerance of mesalamine the active ingredient of lialda, then it should clear pretty quickly within 24-28 hours of discontinuing it.

Usually a mesalamine intolerance is a dramatic worsening of your symptoms, like going from 2 bms a day to 8. Not just a little blood seen.

What exactly happens with your adverse reaction? Is it more than just blood?

What makes you think that Lialda is an NSAID? And what exactly are yhe adverse effects it does to the rest of the body?

@iPoop, I basically started bleeding pretty bad after taking the lialda. That was on Sunday. There's still inflammation and streaking blood, but still just 1 BM per day, and it's Wednesday. I have a doctor's appointment next week to sort this out. I'mean pretty sure that this may in fact be a flare, possibly made worse by the lialda. To go from seeing no blood to having it appear along with cramping and whatnot after takimg it is significant...It's not improving but not worsening progressively either. Also, as far as other adverse side effects, everyone responds to it differently but my liver and kidneys don't seem to be able to handle the maximum 4 tablet dose over an extended period of time. My blood work, (i.e. LFT's and Creatinine get out of sorts).

Lastly, Lialda is a Non Steroidal Anti Inflammatory Drug, albeit thru a targeted release mechanism, so I don't think it's an NSAID, it IS one by definition.

Yeah, from what I read, it's not considered but related through its structure. Its like stating that mesalamine is ASA, which it isn't.....same family salicylate, but not ASA. Taken with NSAIDS, mesalamine can increase problems with kidneys, liver if one is super sensitive to mesalamine or salicylates. It doesnt cross the gut barrier except for a dmall percentage....I assume NSAIDS are classified as systemic.

In your situation....why are you on mesalamines?

Have you had your liver and kidney checked via visual...aka MRCP or ultrasound?

Actually NSAIDs (ASA) share a structural similarity with mesalamine/5-ASA but act very differently within the body. Small structure differences in molecules produce very different results. The inherent similarity means that if you have a bad reaction to one, then there's increased odds you might to the other as well.

Ask your gasteroenterologist about other treatment options, as others are available. There's different classes of medications available that you may not have a reaction to. Personally, it doesn't like an intolerance more like coincidence. But i suppose if this reaction happened multiple times then there is a correlation.
