My routine is that I brush and floss, and then rinse with 2% peroxide. I buy 33% at Whole Foods and dilute it 1 part peroxide to 11 parts pure water (not tap) to make 3%, then I might cut that in half. peroxide kills all of the odor causing bacteria, makes teeth white, and they remineralize faster. It's the only thing that stopped gingivitis, for me. You have to make your own from food grade peroxide because the stuff in pharmacies has phosphoric acid added as a stabilizer, which will corrode your teeth.
If that doesn't help your breath then quincy's right, it's your stomach or your sinuses. You'll know your sinuses are involved because you'll have thick, gross smelling mucus when you blow your nose. If you can't smell your own bad breath then that suggests the problem might be in your nose.
An h. pylori breath test can easily tell if you have it. They give off gnarly gas that makes breath stink.
