I don't have any urgency, mucus, blood etc but I am experiencing rectal pain and pressure. The area aches all the time and it's pretty uncomfortable. If I do need to use the bathroom or just gas, I experience intense pressure pain. I have no idea if this is UC related or maybe hemorrhoids? I do not believe its tenesmus because I don't feel the need to use the bathroom necessarily, it aches all the time. It's much much worse after I do a long session of exercise. ( I am a long distance runner)

I have been using canasa suppositories on top of taking my Apriso and I am a bit better today. Any suggestions?

Does it hurt when you sit? Does it hurt when you bend to reach down and pick something up from the ground? Does it hurt before, during, or after pooping?

We can have intestinal spasms that tend to last all day, it's common with an IBS and they prescribe Bentyl (an anti-spasmatic) to treat it.

We can get internal or external hemorrhoids.

Although less common with an UC, we can get a variety of perianal complications. The major perianal complications include fissures, fistulas, abscesses, and stenosis, alone or in combination, which affect approximately 35 to 45 percent of patients during the course of Crohn's disease (not sure the odds with UC).

You can squat over a mirror and look for any paper-cut like tears on the surface or just inside the sphincter (that is a fissure) or pimple-like bump on the outside but near the sphincter (that's an abscess). You can look for external hemis, they extend outside the sphincter.

Have you tried a sitz bath?

Could be a combo....but having nasty and painful hemmies could spark a flare.

You should have it checked out, but adding the supps...I'd do 2x daily...is a good thing.

If you've recently gone through a constipation, harder stool, skinny stool, small stool....etc, I'd say flare.

I suggest go light on the exertion of exercise. Antispasmodic for sure!

When I had an abscess, pain/pressure grew over the course of 4 days until I could not walk. I then was able to feel a lump that was incredibly sensitive. I wasn't flaring at the time. Rowasa made it worse. UC patients are at a higher risk than the general public. Many "regular" people get them as well...it is just not talked about. This is one possibility.

Thank you for the replies. It doesn't hurt more to sit or bend etc but standing makes it ache the most. After a run I have to keep sitting or it hurts. I can't see anything abnormal when I look other than hemmies so I guess it could simply be that. Just doesn't have the other hemmie related symptoms but with UC who knows. I am feeling better today thankfully! i Plan to take some time off of training after my race Saturday.

I wanted to update that my half marathon was Saturday and it went great! No issues during the race but the pain right after was intense. However, by the next day there was no more rectal pain! I'm still not sure if it was UC related or hemorrhoids but I am glad the pain and pressure is gone!

I slowly developed aching anal pain over a 2 wk. period that became unrelenting spasms along with constipation, hemorrhoids. Came on after BM every morning and lasted for 2-3 hours. Dr. dx: Proctalgia Fugax. Don’t believe websites that say this is “fleeting” pain. It’s not. Seems current proctitis flare with no blood or diarrhea is causing pain, spasms, constipation, bladder irritation, etc. Worse when walking or standing for long periods. My treatment is Rectal nitroglycerin ointment 2x daily and lie down for 30 minutes to relax. Also drinking 6-8 glasses of water daily and increase my fiber. Using Cortenema at night and Canasa in day. Proctalgia Fugax was a new one for me.

Thanks for the info! It doesn't sound quite what I am dealing with but who knows. I need to schedule an appointment with my GI soon. I'm sorry you are dealing with that kind of pain. Its very frustrating!
