Insurance is complex, and varies a lot based on where you live or where you work. If you move around a lot, then try for a nation-wide insurance policy (generally more expensive than a local plan). Many insurances cover only local facilities within the same USA state in which you live, consider that in-network (traveling would incur more expensive out-of-network pricing). Nation-wide plans enable you to use a hospital or doctors office anywhere within the USA, and sometimes a few international countries if you are abroad.
Nearly all cover xeljanz, with pre-approval. Often xeljanz is not a first biologic, others have to be tried and failed first. For example, fail remicade 1st. It varies.
