Anyone on Skyrizi for UC ?

My 29 yr old daughter has had UC for 11 years. She has been on Remicade for 2 1/2 years & doing great until this fall, she developed Psoraisis . She has tried to hang in there using all the creams prescribed by her Dermatologist,( & her UC has been under A plus control on it) but it gets slightly better & then gets very bad again! She is now over it! Her GI Dr mentioned Skyrizi. I was wondering if anyone has any feedback about this drug?

He mentioned it for UC or for the psoriasis? I thought that medication was only approved for psoriasis.

He told her it works well for both. He also said fewer side effects. He is a wonderful GI Dr at University of MD.

Hmmm...well it's not approved for UC or for Crohn's yet. I looked it up. It's in clinical trials: https://www.clinicaltrials.gov/ct2/show/NCT03398148.

Not sure why he would tell you it's a great med with few side effects for UC when its safety and efficacy haven't even been determined yet.

I am not sure either, but that is what she said. It sounds great for Psoriasis though. It is possible he may be doing clinical trials for UC at U of MD. She is making an appointment tomorrow. I will keep you posted.

Why would you do a clinical trial if there are other drugs that currently work for both illnesses and are approved by the FDA? Depending on the trial your daughter could get a placebo or a lower dose.

There are a few other like Simponi, Humira and Stelara that are approved for both psoriasis and UC. Stelara is a different mechanism than the anti-tnf drugs like Remicade, Humira, Simponi and Cimzia.

I think it's the same as stellara, il-23 inhibitor. Worth a try.

UC, Crohn's, Psoriasis, Lupus, RA all intersect at one or the other place in terms of pathophysiology so medication approved for one will work for others too, just not to an extent compared to the one made specifically for other condition.
If Skyrizi is approved for Psoriasis, it will work on UC too, just not to an extent as say Remicade, Humira or other specifically indicated for UC.

Since your DD has both the condition, I STRONGLY recommend, you read up research articles covering both and beyond traditional medicine. Traditional medicine only addresses the symptom, not the root cause. Antibodies are great suppressing symptoms, only until your body develops antibodies against them, which may be tomorrow or after 10 years, YMMV.

Stelara is approved for both, Skyrizi will be eventually too. Maybe he likes skyrizi better. My GI asked the other day if I wanted to try it instead of Stelara. Luckily she qualifies for it because of the psoriasis. And actually some of these IL things work better than tnf but it is different for each person. I guess the gamble is which one will she respond to the most. I don’t respond to remicade etc so no one ever knows. Good luck!

Yeah they're similar.

Stelara blocks the interleukin (IL)-12 and IL-23 cytokines.

Skyrizi blocks just IL-23 cytokine.

Since Skyrizi isn't approved yet for UC she would likely be getting the dosage for psoriasis (unless she is in a trial and gets a higher dose). Based on other biologics, the psoriasis dosages aren't usually high enough for UC.

You might want to ask about Stelara since that works for both and has already been approved for UC

I don’t have psoriasis but before Stelara was approved for CD my GI got me approved for off label use. At the Stelara psoriasis dosage it did not treat my IBD very well, I later was able to get the full CD dosage with the IV loading dosage and it’s been great for me for almost 3 years.

They know that Stelara works well for both UC & Psoriasis at the higher IBD dosage.

She just saw her GI Dr this am & will start Stelara as soon as his office arranges it. He said if he had UC (& she has new severe Psoriasis) he would go on it too. Hoping for the best!
