Very interested in thoughts: What would you think and do

Hi everyone,

This is going to be a bit long. Some of you will know my story but I will start off by saying I have had UC since 2003. Since then, I’ve had mild disease and most scopes showed remission via biopsy.

The furthest extent was 25 cm I believe and mild to moderate. That was 10 years ago.

I did flare after 2 of my 3 children (2009, 2011) mildly. I somehow managed to come out of those flares with staying on my regular meds (Salofalk oral only at those times.)

I have a lot of anxiety surrounding my UC. A LOT. This means I am constantly thinking I’m flaring. Although I have never actually been when I was so worried. I’ve learned a lot though about variation of stool and food, that time of the month and also managing my anxiety. Not to say I still don’t freak out.

I am currently on 9 Salofalk oral( have been the entire time) and a Salofalk enema every third night. I use a Salofalk suppository the second night just because it gives me peace of mild mostly I think.

I had a clear scope in July 2019.

Anyway: I’m having symptoms the past month. I really was thinking hemmi or tear or something.

Here are the symptoms:

-I can feel a little bump on the inside on my anus/rectum with my finger
-some days I was/am seeing a red streak on the last pieces of stool ( minimal)
-no mucous
-solid formed
-normal thickness and shape
-no urgency or tenesmus
-bm once a day In the morning
-as the month progressed my butt got sore and it felt irritated and painful after a bm ( with a red streak or without )
-I have at times felt random sharp pains but only one or two days
-my enema and suppository are somewhat difficult to insert now bc that part is sore and feels like burning and raw
-family doc Rx Proctocedyl ointment and suppositories that I used for 2 weeks and seemed to be better
-although when I used the enema on third night it would hurt and I would possible see a red spot or streak of blood the next morning (reaggrivating something?)
-I stopped the Proctocedyl and I am again seeing blood steaks on the second half of my only bm in the morning.
-Butt is sore
-today I saw more blood that I think I have every on the last few piece of stool. Not in water
-a few hours later went pee and my behind was itchy and irritated feeling so I dabbed a tissue and the was bright red blood dispersed not like a soaking tissue


Ok thank you for reading al that. I have a GI regular appt at the end of the month where I will discuss.

Is this low in the rectum inflammation and should I up all my enemas to nightly and use suppository Salofalk each morning or do I continue on my maintenance regime with those and add in Proctocedyl suppositories and ointment again?

So confused but I don’t want to increase if this isnt a flare. Also the enema really hurts and possibly re irritating what ever is happening in there (fissure hemmi ?)

I agree but I suppose I was getting more concerned as the blood has become more consistent.

Perhaps I didn’t give the Proctocedyl enough time to see or before stopping?

Ok thanks. Yes it is used for Hemmies and fissures. It is the one the doctor prescribes bc it has a little steroid in it. Stronger than anusol and the such.

That helps with my knowledge and your experience sounds like mine. I do feel my bump about 1-2 cm above sphincter. Inside.

I guess I need to stick to a consistent plan of using the correct hemmi meds (as a good friend has suggested a couple times).

The hemmi treatment you use, is it over the counter or Proctocedyl? I’m wondering how long I could use that for. It says 3 weeks but maybe longer?

Appreciate the real talk. I guess I’ll use the Proctocedyl suppositories for a week or max 1.5 weeks and then hope to graduate to anusol or prep H. I’ll continue with my maintenance med regime for my UC as well and keep in mind all you’ve experienced with bleeding Hemmies.

Another question for those who have this: How much blood and how do you see it? On the stool, streaks or covering part, on tp?

I used to have streaks and the last few days it was bright red on my solid stool pieces but more a light coating. The first pieces seemed ok then the rest all had bright red on them mostly on the tips but covering the stool. No blood on toilet paper.

Do you take any kind of fiber? If you have constipation, that would make the hemmies worse. Something like psyllium (Metamucil) can help.

It does sound like hemorrhoids. If you want to know for sure, you should go to your GI and have them look. If they are internal, they may have to do a flex sig. Steroid suppositories could be a good option for you. They are really reasonably priced and easy to use.

I used the Proctocedyl suppository last night and again this morning. The pain seems much less but still some steaks of blood. It’s been 4 days after today of use.

Stool seemed normal firmness and sized and just once in the morning after waking.

I inserted the Proctocedyl suppository and used the ointment about 5 mins after my bm this morning (maybe should have waited a bit) and had some urges to expel it about 30 mins after. As well, later in the day ( about 8 hrs later) I again had the urge to go to expel the melty suppository or bm?

I did not go and do it as I wanted to keep the meds inside. Although I may when I’m home bc it is an uncomfortable feeling.

I had posted a new post about this which was a bit panicky sounding so I deleted it. I do have questions about this though. Maybe the hemmi or Uc experienced can chime in? I’m not very experienced at knowing signs and symptoms and what they mean. I’m starting to learn though.

-Does this sound like tenesmus?

-Does this sound like rectal inflammation?

-should all the bleeding be cleared up 3 days after starting to use Proctocedyl?

I have a GP appointment tomorrow for him to look/ feel. The bump feels actually smaller to me when I touch it if that’s possible ?

I’ve been able to always keep them in but on occasion this happens. Where I feel it melted more or feel like it wants out! I guess this time I was nervous it was inflammation as opposed to just gas or something.

Glad the hemi is getting better, and glad you're not flaring!

I have bright red, obvious blood on my toilet paper. Sometimes drips of blood on toilet water surface. My poops are solid but lot of small bits. So, no chance of seeing streaks on it.

Still sounds like hemis to me...

Thanks for the replies. I find that if i use the Proctosedyl suppository consistently I feel better and the blood goes away. Unfortunately since I use an enema twice a week I re injure the hemmie all the time. Mine is internal so the nozzle is always hitting it no matter what angle. Sometime extra lube on the nozzle helps but not much.

I also have a sore anus after using the enemas and am likely to see blood streaks or red blood on the tip of my stool.

It’s still hard to see the blood, as in the back of my mind I’m wondering if it really is Hemmies but this is now on and off for a couple months so I guess it is. My GP also said that is what it is.

I don’t have the option of stopping my salofalk enemas as I use it as a maintenance med so I guess I just need to accept this.

I have a phone appt with my GI where I will discuss at the end of the month and will update.
