Does this type of bleeding happen only with hemmies?

Hi, so basically Ive been bleeding again for the past couple months. I have pancolitis, last sigmoidoscopy showed moderate 4cm inflammation so I've been on nightly mesalazine foam enemas and morning budesonide for 1 month, still bleeding.

Thing is, I notice blood usually only if I have to push a bit or its a solid biggish BM that on a usual day would feel like a 10/10 but upon wiping, bright red blood on the paper.

Previously my GI has seen I have had hemmies and put it down to that before the sigmoidoscopy.

My question is, if there is an active 4cm flare, would more blood come out or even sometimes drip out when pushing or is that sounding a lot like a hemorroide would do?

I still cant get it round my head how I have 4cm inflammation after 5 months of nightly retention Salofalk enemas and another month of nightly and daily foam ones.

I use enemas for general inflammation/maintenance, but when I see occasional bright red blood like that I assume it's my hemi and I switch to steroid suppositories for a couple days and that takes care of the issue. My theory as to why the suppositories work better than the enema is that the suppositories stay low in the rectum and the enemas go higher so may not be as concentrated where the hemi is.

In the last year the hemi has acted up 2 or 3 times. 2-3 days of using the suppository resolves the issue.

Yeah I was thinking about that also, that perhaps the suppositories would coat that area better. Ill bring it up at my appointment tomorrow.

Been on the budesonide foam though which is the steroid for a month and still bleeding but maybe the same, its staying further away than where the issue is.

You can get those steroid suppositories for just 7 days. That should be long enough to see if it resolves the blood. If it works then see about getting some to keep on hand because hemies will flare up occasionally

Would I be right in thinking that it could in theory help both problems, both the 4cm inflammation and the hemmie in that case?

Not sure why, but I assumed hemorrhoids had to be fixed some other way, I didn't realize they can just stop with steroids.

Hemis bleed when they get swollen and then get irritated. The steroids calm them down so they aren't swollen. To get rid of them permanently requires banding or surgery I think.

Yes the suppositories might help with such a small area of inflammation

The other thing about enemas is that if you have a swollen hemi you could be scraping it with the nozzle which could be causing the bleeding

I see what you mean and YES I thought exactly the same about the nozzle because I had been going for a couple of weeks without blood and I'm sure it started again after an uncomfortable time inserting the nozzle almost as if I scraped something by accident..

From my personal experience, the Mesalamine enema is irritating my hemi, I believe one of the ingredients, not the nozzle.
Recently I switched from Canasa supp to Generic Mesalamine supp and my hemi flared again!

So Ive been on 1g salofalk enemas for 10 days, once daily and once nightly and this morning again when going for a BM, its solid but wow..loads of blood, whole water turned red and there was dripping.

My recent calprotectin was 400 so I know I do have some inflammation but I get the feeling this bleeding is a hemmie or similar because:

1. I usually see the blood at the end of the BM if its on the stool, I think perhaps of pushing a little
2. I see blood mainly when I have to push a bit, this is when I see dripping.

Would pushing cause dripping of blood from the 4cm flare I seem to have? Seems like a lot of blood and very fresh, also of course the toilet paper is covered.

I don't have any other symptoms aside from the large amounts of blood.

The dripping would most likely be from something else...hemmie or fissure.
Ask your GI to send you to an anorectal specialist or ask him specifically to put you on meds to treat them. So frustrating for you to be dealing with this.

In the meantime...add foods that will soften your stool. If I have to do it quickly I increase vitamin C supplement intake.

I think I may be scraping something with the nozzle of my enemas, maybe a hemorrhoid, because I see bright red blood afterward, and there's a bit of pain. I want to try some Preparation H, but I'm not sure when. I use a mesalamine suppository in the morning and an enema at night. So when should I use the Preparation H?

MM, do you have the ointment? If yes, use it after your suppository and the enema.

Hi, quick update.

So last Friday I went to see a anorectal surgeon. He had a look with some instrument and said that it is not hemorrhoids but a wound likely from inserting the enemas, he said I was lucky and it could have been a lot worse...

He said it should resolve on it own now that Im not on enemas, only suppositories.

Thing is. He wanted me to take Flagyl antibiotic for 7 days in case it got infected..

Well its the 3rd day and I feel terrible, diarrhea 4 times today, Im hoping it is just the antibiotics and not kick starting something like a flare that I am already treating...

I have to wait until like after midday to put the morning Salofalk suppository in otherwise it’ll come straight back out.

Does this procedure he said sound normal??

Just a note on flagyl. It’s the antibiotic of choice for colon issues but I do horrible on it and refuse to take it now. It gives me diarrhea and nausea so bad I can’t eat — not to mention the constant horrible taste in my mouth and then I get very emotional and depressed to the point that I feel hysterical. So my guess is the diarrhea is from the flagyl. (Most people don’t react to it the way I do. )

Good you went to have it looked at.
I've never used flagyl.

I'm wondering if there is a topical antibiotic that could be used in the anus instead as another option?

Well, 3rd day after stopping the abx, and yes, no blood but 4-5 times a day diarreah, like opening a tap... I do hope this resolves soon.

Im following the BRAT diet for a few days to see if that helps...

Do you have probiotics? Add some fibre supplement with meals...should help to bulk a bit and slow it down somewhat.

I started taking the probiotic called Prodefen Plus during the course of abx and Im still taking 1 sachet daily. Im not taking any fibre supplements. I assumed that would just make me go more.

I'll get some.
