I'm on Remicade and I've had surgery 4 times now.. and depending upon how I am doing health wise we have done different things.
We try to schedule it at least 3 weeks after my infusion.... And they try to have me wait at least another 3 weeks after surgery before I have an infusion. When I developed an infection after one of my surgeries we delayed it to almost 3 months after the surgery which was already 4 weeks after my infusion. You can't fight off those infections so you need to be careful...
The infection was internal... I developed chills at night, felt lethargic, and my WBC just increased a little above normal... But they were not taking any chances.
I did develop antibodies to Remicade... We checked when we started doing labs for the infection.... I believe I was at 5 weeks after infusion at that point... Antibiotics for a week... Still infected... IV antibiotics for 2 weeks then.... Waited another 2 weeks and then started me in larger doses of methotrexate to drive down antibodies.... Used premed, did infusion over 5 hour period.... Still had some reactions so I took Benadryl every 4 to 6 hours for another period of time after infusions. We increased the amount of Remicade, decreased time between infusions, got trough level up to about
30? And then we started to slowly use less Benadryl after infusions and then we backed down the methotrexate amount. Methotrexate doesn't negate Remicade antibodies for everyone but it does for some. It's done it for me 3 times now ...
BUT.... On my kidney surgeries he went in 4 days after infusion and I had my next infusion at the 4 week mark... AND he didn't give me any antibiotics after surgery. He was a young doctor and he showed me studies that indicated that it's ok for most patients... I was very uncomfortable that time... And the next time he did it again.... But I didn't get any infections.... But boy ..I was so watching for one..
I grilled the kidney doctor on his studies and even called my CRS and GI and PCP..... Only when my infectious disease doctor told me that things have changed and that it should be fine was so ok....
