Help with cortifoam - Quincy?

Okay, I'm not really loking for advice about cortifoam specifically, but about daily enemas in general.  I just didn't want to put enema in the subject line...lol.

My doc had me start on Colocort enemas last week, and I've been having a heck of a time with them.  I'm supposed to do them twice a day, but I end up gassy and feeling like I'm leaking after using them.  This has made me only willing to use them at night, because I'm not going to be farting and leaking at work.  Is there a trick to the injection process?  Any advice about this will help me out.

Hi...you say cortifoam in the title and colocort in the posting...i'll assume it's the Colocort liquid you're meaning?

Basically, one should lay on the left side, lube the tip, insert it as stated on the directions and slowly squeeze the bottle.

Lay there for a half hour.

Maybe you could get the doc to prescribe you the proctofoam, which I would think is much easier to retain. Also, it won't be so much steroid med.

Are you still on the pred?

Do you have any problems using them at night?

Quincy if my asacol suppositories don't work - what next? Shall I ask for steroid rectals or shall I ask for enemas first?

Just to be clear about proctofoam vs. cortifoam -- cortifoam is 10% steroid, 80 mg; proctofoam is only 1% steroid, so I assume 8 mg. I used proctofoam for a couple of weeks and got worse. But maybe used in conjunction with a cortenema at night it would work better.

LR...how many times are you using the Asacol supps? You can use them a few times a day...at least you could up the dosage to twice daily.

I am using 1000mg at night. My doc says that's a high dosage for one day.

He also said that I don't show the physical symptoms of IBD - weight loss, feeling unwell, loss of appetite, etc, and don't show the blood work or pathology of an IBDer. I only show the bleeding and scope pictures - which is why he wants to move slow with me and cause further bodily responses just in case it makes it worse than it is.

I'm not using cotrifoam or proctoform. What I've got is Colocort, a hydrocortisone rectal suspension solution. Definitely a liquid. 100mg hydrocortisone in 60ml of water essentially. I'm supposed to use half a bottle at a time, monrings and evenings. I've been following the directions, which are basically the same thing Qunicy told me. How does one keep from blowing air in their butt? I'm able to retain the liquid for the half an hour, but seriously, even after I expel it, I feel like I'm leaking until I go to bed. Due to this feeling, I used it once in the morning, but haven't on any day before work. Is the leaky feeling normal, or am I just paranoid because I've had leaks (accidents really) before?

Qunicy, yes I am still on the pred, but tapering down. I'm down to 15mg a day now. A bit faster of a taper than my doc wants, but I'm really keen on staying on it. I'd rather get the enema thing down, and get off the pred.

Brandon - I use Rowasa which is basicly the same consistency enema.  I don't believe I could leave it in 1/2 hour and then get out of bed.  I can only get 1/2 or so in at once (doc said that was ok) then I lay still for an hour on my left side if I can - resisting the urge to let ANYTHING OUT - white knuckle it (as I believe quincy once said).  I then keep it in all night and usually expell some gas when I move around.  By the way, squeeze that bottle and let all the air out until you see medicine before you insert.  I have to do it on the toilet because of obesity and arthritis but works ok. I make it quicklly to bed!

Maybe you could use the enema at night and suppository daytime as they are easy to keep in.

Thanks for the tips. I never thought of squeezing the air out prior to, so I did that tonight. Seems to have helped on the air injection. I still feel like I'm leaking something though. Do you ever feel like that?

Yes, Brandon - but being a woman (granny woman) I wear a pad - don't seem to find much there but feels like it - I know what you mean.  I have never tried it during the day- always sleep with it in there.

Geez, I had a whole long post to this that got lost somewhere.  Maybe it was too long, HA

Brandon, are you actually leaking or feeling like you are?  Could it be gas? 

I'm using generics, but I noticed that I can get air out of the gen rowasa, but not the gen cortenema.  When I squeeze the bottle, liquid comes out almost immediately, making it messy to use. 

Can you use an antispasmodic an hour to 1/2 hour before using the enemas?  1/2 lomotil is works well for me.

You might look at Dr. A's response to Hans on spacing out his enemas... I thought it made a LOT of sense with good suggestions.  I'll try to find it and bump it up.

I suspect as you heal this will get a lot easier and more comfortable for you.
