Hi....hydrocortisone ointment over the counter from the pharmacy is good as well as zinc oxide.
Using a fibre supplement will help A LOT....keeping the stool softer/fluffier, easier to expel and not tend to open the fissure as readily during a bm.
Mind you, for me, it's the wiping most times that opens mine....and once the external hemmie "rears"...then the discomfort is tenfold.
My fissure is small, but bleeds a lot if ripped open. It doesn't normally hurt.
They definitely are in degrees of severity, and the goal would be to treat asap with anything that will help the inflammation and promote healing. Keeping them clean is key, any miniscule amount of stool left in them can promote more inflammation/discomfort or infection.
