Managing PAIN

If the hemmeroids are hurting this severely, have you asked the doc to have them removed? Have you ever tried a steroid cream to help reduce the size of them? I have one that is partially internal and external and I use a hydrocortisone cream to help shrink mine. It works fairly quickly. Mine are more of a nuisance then anything but occasionally I will have a sharp pain from them. I couldn't imagine being in that type of pain long term.

If it turns out not to be hemmies please let me know as I have some info that could help you.

Depending upon how you treat it and your bowel habits it could be a few days to a few weeks, they key is not letting it become chronic.
1) Dont strain and breathe out through your mouth when passing a stool...this help relieve pressure in your sphincter and lessens the chances of a fissure or making it worse
2) Softnen your stool (if its firm) try colace or miralax
3) Take long warm/hot (but not too hot) baths with some epsom salts
4) Use wet wipes for cleaning yourself and be gentle
5) The doctor may rx some creme that's either cortisone based or one that increases blood flow to the anal area which helps the fissure to heal 
6) Add soluable fiber to your diet ie Benefiber
7) Drink plenty of water
8) If you home has a bidet use it after each BM and sit there for awhile...water is key to helping the fissure heal
9) Make sure for any future rectal exams you tell the doctor to be aware not to reopen the fissure
10) Tension in your pelvic floor can also cause fissure so try to relax

I feel your pain - I had four fissures earlier this year and it was the most intense pain I have ever experienced. My doc gave me nitroglycerin cream compound which gave me headaches, but worked, so I dealt with the headache. He also gave me pain meds (darvocet) which helped me get through the day. Warm sitz baths help a lot too, but of course the best medicine is pain. All the best!
