Saw GI today and he showed me a picture of my perfect colon above the mildly inflamed rectum with slight granularity. What a difference in images.

Anyway, he seemed to say that maybe I didn't have pancolitis like I was first diagnosed. (I'm sure the initial diagnosis was accurate, but he says maybe not) Anyway, is it possible that if the rest of your colon is picture perfect, and the rectum is not, then you could NOT have pancolitis.

I'm thinking I have pancolitis and it's wishful thinking to think not.

Also, GI was pleased with my case management. Said his goal was to have me know my disease better than he does.

Well, his goal may have already been met! I don't mean to burst your bubble, but is this colonoscopy after some treatment? Could it be that the rest has healed and the rectum is the last to heal because you're treating from the top down? If a previous colonoscopy showed pancolitis, it seems weird to me to change the dx after tx has been mostly effective. That would mean I don't have UC if I'm in remission. That logic is wrong.

I agree with your logic completely.

A colonoscopy diagnosed pancolitis and that was well over a year ago and it was a different doc. He has only done two flex sigs on me not a colonoscopy. Impossible, that my pancolitis just disappeared except for the rectum. It's just stubborn there and right now the mild inflammation has no effect on me. I'm hitting the rectum with enemas every other night. So far so good.

Oh shoot, I'm taking a plane tonight and can't do my rectal.I'll stick in a suppository!!

I would guess that if I had a s/c-scope right now, I'd either have no inflammation or only very limited inflammation in the rectum (but I doubt even that given my lack of symptoms). But I think that if a doc were to do a scope when I first enter into my next flare, he wouldn't find pancolitis. I believe that flares can vary in the amount of the colon that's affected. Sure, since I was diagnosed with pancolitis, I'm more likely to have future flares that involve the entire colon, but that doesn't mean my entire colon will always be inflammed with each flare.

I have this fear that every time my mild rectal inflammation kicks up a notch it will automatically whip up the colon and quickly -- unless I nip it in the bud. I don't want to take any chances on a full flare. Containment is my policy, but it is a comforting thought to think that it might just stay in the rectum even if I have pancolitis.

As I mentioned before, my flares are limited to the rectum, even low rectum...I was initially diagnosed with proctosigmoiditis.

You should be covered with the oral 5ASA...an deal with the lower with the rectals.

Welcome to phase 2.

Now...if you quit your oral 5ASA....your UC might creep up again.

There is logic to the philosophy.

Wish my doc could read these threads. He's so off base with this. Glad to know the Asacol is hard at work!
