Does your PCP nickle and dime you?

I have a PPO plan and my PCP nickles and dimes me for addtional stuff...like signing a flex reimbursement form I filled out for him - $5. Mail order pharmacy requested additional information so I get the correct prescription - $20.

This is rediculous!  It's not like I'm choosing to play in a sport after school and need a sign health care form or I want to have a certain prescription when there's mutliple manufacturers!

Any one else having issues like this?
