I had have had two vaginal deliveries and also went without pain meds for both labors, so just because you have UC doesn't always mean interventions like a C-section. Ask your GI what inflammation has to do with pushing out a baby and then talk to your prenatal health care giver. I was on prednisone for my whole 2nd trimester with my first baby, so definitely flaring then, and they just watched me a lot(the pred causes all kind of side effects when pregnant, like diabetes, high blood pressure, etc) and though they did induce my labor at 40 weeks and a couple days to due low amniotic fluid, there was no talk about a C-section at any time. Good luck to you and definitely get informed the best you can, my experience was that the GI didn't know a lot about OB/Gyn and my OB/midwife didn't know a lot of about GI issues, so I had to mediate and do what I ultimately thought was best, which included lots of questioning both providers about interventions and medications.

I was advised by my GI and OB to have a c-section if flaring at the time of delivery. Their reasoning had to do with the possibility of any tearing into the rectal area. Apparently, if this happened it could be extremely complicated to heal while trying to treat a flare. They told me the risk was small, but then I ended up with a large baby so went for the c-section. It wasn't bad. I recovered pretty quickly. I agonized over this decision, but in the end was just so happy to have my healthy baby, it made no difference how she got here. Good luck.
