If you have UC, it doesnt matter where your disease js active, you would normally still be eligible for an internal jpouch and no external pouch a.k.a. "bag". (Long Sidebar: I was and still am eligible for a jpouch but actually prefer "the bag" after discovering it is no big deal at all-- a synthetic, disposable pouch can never get any sort of "-itis", "butt burn" or urgency, has no nerves, and has a "wait until its convenient" poop time of 6-8 hours or so if I ever need to.. But I digress lol. Oh, and its ileostomy not colostomy in the case of UCers- colostomies are external pouches with some part of the colon still functional).
As far as avoiding surgery, I would certainly give biologics a shot if you havent yet. For some they are the bomb. I had a ten year super-remission on no meds and en nothing, inlcuding biologics, FT, worms etc gave me that again but I could never tolerate 5ASA's. Since you can, your chances of avoiding surgery and getting long remissions are good methinks. However you can and should get a surgery consult for peace of mind- no surgeon will pressure you for surgery, but getting "pre-authorized" can relax you by giving you a get out if jail free card in your pocket. Just check back with the surgeon (or a different one) every couple years or so tops if you do t opt for surgery to keep that "pre-authorization" active, as I discovered, otherwise the authorization can go stale and they'd insist on a new consult orior to scheduling.
